Abstract

AbstrakKesalahan medikasi merupakan masalah yang sangat serius di pelayanan kesehatan di seluruh dunia. Masalah tersebut mengakibatkan cedera dan kematian bagi pasien, serta meningkatkan biaya yang harus dikeluarkan oleh rumah sakit. Kesalahan medikasi dapat terjadi di setiap tahapan proses manajemen dan penggunaan medikasi dan berakibat pada keselamatan pasien. Kesalahan medikasi dapat terjadi akibat kondisi laten, kondisi yang menyebabkan kesalahan, dan kegagalan aktif. Perawat sebagai pihak yang paling banyak terlibat dalam proses pemberian medikasi memiliki peran penting dalam mencegah, mengenali, dan mengatasi terjadinya kesalahan untuk meningkatkan keselamatan pemberian medikasi. Upaya meningkatkan keselamatan pemberian medikasi dilakukan melalui pendekatan proses keperawatan sejak pengkajian hingga evaluasi dan dokumentasi. AbstractMedication errors are a very serious problem in health care services around the world. These problems lead to morbidity and mortality for patients, as well as increase the costs to be incurred by the hospital. Medication errors may occur at any stages of medication management and use process and result in patient safety. These may occur due to latent conditions, error producing conditions, and active failures. Nurses who are primarily involved in the process of medication administration have important role in preventing, recognizing, and addressing errors in order to enhance safety medication administration. Efforts to enhance safety medication administration may be done through nursing process approach from assessment to evaluation and documentation.

Abstract

 AbstrakPerencanaan asuhan keperawatan merupakan kunci dari continuity of care.Dokumentasi perencanaan asuhan keperawatan yang tidak saling berkesinambungan dalam rekam medis berdampak pada kualitas asuhan pasien. Tujuan studi ini adalah menganalisis penyebab fenomena ketidaksinambungan tersebut sehingga dapat memberikan rekomendasi untuk perbaikan bagi pelayanan asuhan keperawatan. Metode yang digunakan adalah studi kasus dengan pendekatan analisis gap antara kondisi yang terjadi dengan kondisi ideal. Studi ini menganalisis fenomena berdasarkan pendekatan fungsi manajemen keperawatan dan metode Ishikawa. Teknik pengumpulan datanya adalah dengan cara wawancara, observasi dan data sekunder dari studi dokumentasi data rumah sakit. Responden dipilih dengan cara tehnik purposive sampling sejumlah sembilan orang manajer keperawatan. Hasil analisis Ishikawa menemukan penyebab utama adalah lembar POC pada awalnya diberlakukan oleh pokja akreditasi rumah sakit sehingga setelah penilaian akreditasi selesai keberlanjutannya terhambat. Selain itu ditemukan juga masalah lain sepertipengetahuan perawat tentang metode penugasan perawat primer, ketenagaan dan supervisi rekam medis POC yang belum optimal. Kesimpulan dari analisis fungsi manajemen keperawatandiperoleh bahwa fungsi pengarahan dan fungsi pengendalian perlu ditingkatkan agar dokumentasi perencanaan asuhan keperawatan lebih optimal. Keyword: metode ishikawa, perencanaan keperawatan, fungsi manajemen. Analysis of Discontinuity of Nursing Care Planning Documentation: Ishikawa Method.AbstractPlanning nursing care is the key to continuity of care. Documentation of non-mutually sustainable nursing care planning in medical records has an impact on the qualities of patient care. The purpose of this study is to analyze the causes of the phenomenon of discontinuity so that it can give recommendations for improvement for nursing care services. The method used is a case study with a gap analysis approach between conditions that occur with ideal conditions. This study analyzes phenomena based on nursing management function approach and Ishikawa method. The data collection technique is by interview, observation and secondary data from hospital data documentation studies. Respondents are select by a purposive sampling technique with nine nursing managers. Ishikawa's analysis found that the main cause was that the POC sheet was initially applied by the hospital accreditation working group so that after the accreditation assessment finished the sustainability hampered. In addition, other problems were also found such as nurses' knowledge about the primary nurse assignment method, workforce and supervision of POC medical records that were not ideal. Conclusion of analysis of nursing management functions is that the directional function and control function need to improved so that the planning of nursing care is more ideal.Keyword: ishikawa analysis, management functions, nursing care plan.

Abstract

Seorang perawat dituntut untuk memiliki kompetensi kultural sehingga memiliki kepekaan terhadap kebutuhan budaya pasien. Artikel ini bertujuan untuk memberikan gambaran peningkatan kompetensi kultural perawat yang masih rendah melalui program Pelatihan Asuhan Keperawatan Peka Budaya pada Pasien dengan Gangguan Respirasi. Desain yang digunakan adalah quasi eksperimen dengan pre dan post test tanpa kelompok kontrol. Program pelatihan ini berlangsung selama 4 minggu melibatkan 93 orang perawat pelaksana dan manajer asuhan keperawatan di sebuah rumah sakit di Jakarta Timur yang dipilih secara purposif. Instrumen kompetensi kultural yang digunakan adalah instrumen yang dikembangkan oleh peneliti pada penelitian sebelumnya. Hasil pengukuran kompetensi kultural perawat sebelum dan setelah pelatihan menunjukkan perbedaan yang bermakna. Kompetensi kultural ditinjau dari aspek  pengetahuan budaya memiliki nilai p< 0,001 (?= 0,05), dari sikap budaya nilai p< 0,01 (?= 0,05),  dan dari aspek keterampilan budaya nilai p< 0,001 (?= 0,05). Kesimpulan yang dapat diambil adalah bahwa kegiatan Pelatihan Asuhan Keperawatan Peka Budaya efektif meningkatkan kompetensi kultural perawat dalam memberikan asuhan keperawatan. Rekomendasi untuk kegiatan selanjutnya adalah perlunya mensosialisasikan model Asuhan Keperawatan Peka Budaya yang digunakan dalam pelatihan ini kepada seluruh perawat agar dapat diterapkan kepada seluruh pasien dengan berbagai gangguan kesehatan. Kata kunci: kompetensi budaya, pelatihan perawat, asuhan keperawatan, peka budaya ABSTRACT Culturally-sensitive nursing care trainings effectively improve nurses? cultural competency. Nurses are required to have cultural competency in order to provide culturally sensitive nursing care for their patients. The purpose of this study was to improve nurse cultural competences through A Training Program of Culturally Sensitive Nursing Care for Patients with Respiratory Health Problems. The study utilized quasi-experiments design without control. The four-week training program was organized involving 93 nurses at Persahabatan Hospital Jakarta who were selected using purposive sampling. The nurse cultural competency instruments modified by the author from her previous work was used. The results of this study showed that after the training program, the nurse cultural competency increased significantly (p< 0.001 for cultural knowledge, p= 0.003 for cultural attitude, and p< 0.001 for cultural skills). It was found that the training program was effective to increase every aspect of nurse cultural competency. In conclusion, the nursing care training program was effective to increase nurse?s cultural competency. Further programs are needed to improve the program outreach for all nurses and to be applied to the patients with a variety of health problems. Keywords: cultural competency, nurses training, culturally sensitive, nursing care

Abstract

Outpatient units have high activity and interaction, which increases the risk of neglecting full documentation of nursing care. This study aims to analyze factors contributing to the lack of optimal documentation of nursing care in an outpatient unit. This study employed a fish bone analysis approach to identify the root of problems of documenting nursing care in an outpatient unit. This research was conducted in an outpatient unit of the Children's and Mother's Hospital in Jakarta. The data collection techniques of this study were questionnaires, observations, and interviews with the head of the room, Clinical Instructors, implementing nurses, Case Managers, and Nursing Fields. The analysis reveals several results. Nurses, clinical instructors, activities in high work environments, as well as policies and tools for assessment and supervision are inadequate. The absence of effective systems and mechanisms for supervising nursing care documentation and manual documentation systems contributes to the lack of optimal documentation of nursing care in the outpatient unit. Documentation of nursing care extremely depends on the workforce, work climate, sets of policies, systems, and facilities. This study recommends programs and supervision activities for outpatient nursing care documentation performed by the Nursing Division, head of rooms, and Clinical Instructors, arranges supervision tools, arranges patients? effective and efficient assessment documentation according to accreditation, policy re-socialization and documentation techniques, as well as energy management and implementation time documentation of nursing care in an outpatient unit.
