Abstract

Cancer has many side effects on patient?s physic and mostly on mental which may affect on their quality of life. Physically most patients feel fatigue, insomnia, anorexia, nausea, and vomit while mentally they feel emotional disturbances such as anxiety, stress, or depression. One of the methods to that appropriate to overcome both of the conditions is spiritual care. Many research found that because spiritual care associated with better wellbeing, less emotional disturbance, less substance abuse, greater social support, better health behaviours, so it has impact to longer survival periods. This research aimed to investigate nurse?s spiritual care competencies to patients with end stage cancer. This was descriptive study design employed Spiritual Care Competencies Scale (SCCS) instrument. A total sampling of 24 nurses working in Oncology department in Dr. Wahidin Sudirohusodo Hospital were recruited. This research found that there are 12 nurses (50 %) that work in oncology department have low competencies of spiritual care. Involving spiritual care is associated with better wellbeing and longer survival periods. Therefore, it is necessary to increase the competence of nurses on spiritual care in meeting the spiritual needs of patients with end stage breast cancer. A training of spiritual care can be an alternative for nurses to improve spiritual care in hospitals.

Abstract

Introduction: Diabetes mellitus (DM) is a global disease endemic and causing of 4.6 million deaths in the world. The Indonesian government and health insurance [BPJS Kesehatan] formulate a Chronic Disease Care Program [Program Pengelolaan Penyakit Kronis (PROLANIS)] for type 2 diabetes that aimed at controlling the glycemic status and the risk factors of macro and microvascular complications. The purpose of this study was to analyze the correlation between the implementation of PROLANIS and fasting blood sugar, HbA1c, and total cholesterol levels in patients diagnosed with type 2 diabetes at Antang and Pampang community health centers, Makassar, Indonesia.Methods: This study used descriptive correlation design with cross sectional study approach. Subjects were 40 patients diagnosed with type 2 diabetes who joined PROLANIS at PUSKESMAS Antang and Pampang, Makassar, with sampling technique used was total sampling. The data were analyzed using correlation test to assess the significance (p), the direction (+/-), and the strength of the correlation (r). The implementation of PROLANIS was measured by using the observation sheets developed based on BPJS Kesehatan criteria, while the fasting blood sugar, HbA1c, and total cholesterol levels were checked by laboratory.Results: The mean of the implementation of PROLANIS was 15.05 (SD ± 5.62), while the mean levels of fasting blood sugar, HbA1c, and total cholesterolwere as followed: 191.80 mg/dL (SD ± 85.15); 8.4% (SD ± 2.08); and 192.87 mg/dL (SD ± 45.07). Using the Spearman's rho test, the study result showed that there was a significant and negative correlation between the implementation of PROLANIS and the levels of fasting blood sugar (p= 0.001; r= -0.724), HbA1c (p= 0.001; r= -0.870), and total cholesterol (p= 0.029; r= -0.35) in patients diagnosed with type 2 diabetes at Puskemas Antang and Pampang, Makassar.Conclusion: The optimal implementation of the PROLANIS is very effective to control the levels of fasting blood sugar, HbA1c, and total cholesterol in patients type 2 diabetes.

Abstract

Background: Various efforts are made to maintain and improve the quality of nursing services, one of which is to carry out nursing care according to the standards of nursing practice safely, effectively and ethically. To maintain quality, it is necessary to control nursing care in a directed and planned manner. One of the controls in nursing management is supervision. Purpose: The purpose of this literature study is to identify the effectiveness of the implementation of supervision on the performance of implementing nurses in hospitals. Methods: Database searches used in this literature review are Pubmed, Science Direct and Google Scholar published in 2007-2018. Results: From the six articles used, it was found that supervision had an impact on improving nurses' performance in the form of actions and behaviors that could be measured to maintain and improve the quality of health services for both sick and healthy patients. Other impacts among them commitment, employee attachment, competence and individual quality as well as improving service qualityto them after completing the credentialing process as stated in the Nurse Clinical Assignment Letter. Conclusion: Implementation of supervision motivates nurses to be more attached to work, and has better performance opportunities

Abstract

Background: One of nursing management functions is organizing nursing staff through assessing their working performances. Managing nursing staff in order to improve their working performance is vital. Thus, it needs to be well-designed to meet the patients? expectation on health care quality. Nurses? performance is defined as the action, achievement, or fulfillment of nurses? responsibilities based on their tasks that have been assigned to them. Purpose: The aim of this study was to describe the nurses? performance assessment and the nurse activities based on nursing clinical authority at the Dr. Wahidin Sudirohusodo hospital Makassar. Methods: This research is a qualitative descriptive study. Data were gathered from Focus Group Discussion (FGD), individual interviews, and document analysis. Results: The study results showed that generally the assessment of the nurses? performances at the Dr. Wahidin Sudirohusodo hospital has been conducted since 2015. However, there were still some areas that need to be improved, such as the quantity indicator assessment that is currently not developed based on the nursing clinical authority. In fact, the assessment of nurses' performance for quantity indicators should be adjusted to the nursing clinical authority that has been assigned to them after completing the credentialing process as stated in the Nurse Clinical Assignment Letter. Conclusion: The assessment of nurses? performance should be adhered to the nursing clinical authority and it is better if hospital managers may synchronize the nursing clinical authority with the online nursing logbook provided in the Hospital Information System

Abstract

Introduction: Chronic pain is an unpleasant sensory and emotional experience with actual or potential tissue damage, or is described as a damage lasting more than 3 months with heavy or moderate intensity. The patient's response to the pain he experienced was done in different ways such as shouting, grimacing, crying, etc. Repair of pain is likely to occur more quickly when psychological intervention is included in medical care because it requires alternative and complementary treatment to overcome the pain including prayer , humor, hypnosis, and group support. Growing evidence-based nursing practice shows progress in helping patients deal with the pain. One of the evidence-based nursing practices is humor therapy. Method: The literature search was done through searching the results of scientific publications 2010-2018 using several databases, such as Such as Google scholar, PubMed, DOAJ, and Cochran, with keyword "Humor Therapy and Chronic Pain. Result: Obtained 6 fulltext articles, were obtained with various methods used, such as quasi-experimental pre- and posttest control group design, randomized two-group review literature, concluded that humor therapy proved effective in reducing chronic pain in all ages. Conclusion: The humor therapy is effective in reducing chronic pain in patients of all ages, both children, adults and the elderly, besides humor therapy can also be used to increase endurance and quality of life in elderly who are cared for in nursing homes

Abstract

Introduction: Acute care is a type of time-sensitive care, this is due to an acute condition requiring rapid intervention. Acute conditions that exist in the inpatient unit would require special care in handling. This acute condition can affect many things, one of which is the patient satisfaction. With proper treatment for acute cases, the course will improve patient satisfaction itself. However, to date, acute care has been poorly defined and inadequately supported in most developing health systems. Aims of Study: This research aims to determine nurses? perception on the implementation of inpatient acute nursing care in the Hasanuddin University Hospital. Method: This research was a qualitative method, data collection using individual interviews against ten participants (inpatient nurses) and validity checks of the data that utilizes something beyond the data. Data analyze used thematic analysis. Result: The results of the research yield four themes. Those four themes are perception of acute conditions, supporting factors in the management of patients with acute conditions, inhibiting factors in the management of patients with acute conditions, and nurses? hope for acute care. Conclusion: From this research it is known that the incidence of acute conditions in the inpatient unit of Hasanuddin University Hospital less than chronic conditions. To handle acute cases, nurse in the inpatient unit take action early by monitoring vital signs, then immediately gave emergency action if necessary, while collaborating with the doctor. Supporting factors for nurses is the availability of emergency trolly, as well as cooperation among nurses who are good. Whereas the inhibiting factor is the number of nurses who are still lacking and the absence of code blue team. Advice from researchers, to health institutions is expected to be able to form a rapid response team (code blue), so that patients with serious/acute condition can be handled more quickly and efficiently, so that the implementation of nursing, especially nursing acute to run well and the nurse also will not experience in serving patients overwhelmed.

Abstract

Nowadays, the demands for health services are increasing, health problems are increasingly complex, the development of science and nursing technology is increasingly sophisticated, and the requirements of the workforce are increasingly demanding nursing staff to be competent. Therefore, nursing education institutions are required to be able to prepare graduates who are competent and able to compete both nationally and internationally. In education field, one of efforts of the Association of Indonesian Nurse Education Center (AINEC) to developing competent Ners graduates is by applying a preceptorship guidance model. But there are still a few articles that discuss the method of guidance in the preceptorship that can be used by preceptors to improve the  clinical performance of student. The aim of this literature review is to identify the method of guidance in the preceptorship that can be used by preseptors to improve the clinical performance of student. This literature review examines the methods in the preceptorship that are used to improve student clinical competencies (decision-making abilities, communication skills, practical skills, and social skills). The article search was carried out on databases Pubmed and Google Scholar, using  keywords preceptorship, nursing students, clinical learning, clinical performance, decision making abilities, communication skills, practice skills. The number of articles reviewed was 15 articles. In preceptorship, to improved the student ability of decisions making can by using case discussion methods, bed side teaching, nursing rounds, and pre and post conference. Communication skills can be improved by case discussions, rounds, bed side teaching, role modeling, and pre and post conferences. Student practice skills can be improved by the method of bedside teaching and demonstration. And social skills can be improved by role modeling. Based on this literature review, it can be concluded that the guidance methods used in the preceptroship are useful for improving the competency of student clinics. Therefore, it is important for the preseptor to know what methods can be used in guiding according to the competencies that are to be improved on students.

Abstract

Pendahuluan:Keputusan klinis yang dibuat perawat harus didukung dengan data yang akurat serta lengkap dan sistim yang baik. Namun dalam pelaksanaan perawatan pasien, pembuatan keputusan klinis oleh perawat tidak didukung dengan sistim pendukung keputusan klinis di rumah sakit.Tujuan:untuk mengetahui sistim pendukung keputusan klinis yang dapat mendukung pembuatan keputusan klinisoleh perawat bagi pelayanan pasien di rumah sakit.Metode: Pencarian sistematis dilakukan dibeberapa database yaitu PubMed, ProQuest, dan Science Direct dengan kata kunci Support SystemClinical Decision AND Clinical Decision Making AND Nurses. Kriteria inklusi yaitu publikasi dalam 5 tahun terakhir dari 2015 sampai 2019,  full text, membahas penggunaan sistim pendukung keputusan klinis oleh perawat, semua metode penelitian baik kualitatif, kuantitatif, metode campuran, kohort maupun survey dan menggunakan bahasa inggris.Hasil: Diperoleh enam artikel penelitian sesuai dengan kriteria inklusi Terdapat dua studi tentang pengembangan sistim pendukung keputusan klinis, dua studi tentang evaluasi efek dan pengalaman perawat dalam menggunakan sistim pendukung keputusan klinis, satu studi tentang akurasi diagnostik sistim pendukung keputusan klinis dan satu studi tentang hambatan penggunaan sistim pendukung keputusan klinis. Diskusi: Sistim pendukung keputusan klinis (CDSS) yang dapat digunakan untuk mendukung keputusan klinis perawat adalah NEWS dan metode refleksi Tanner akan tetapi harus terlebih dahulu diuji coba dan disosialisasikan terkait manfaat serta cara penggunaannya kepada perawat, harus aman bagi pasien dan tidak merugikan pasien serta rumah sakit.

Abstract

Objective : Clinical pathway is a process that involves multidisciplinary health professionals focusing on patient care in accordance with procedures on an ongoing basis, and with the application of clinical pathways in hospitals will have a plan in treating patients so that services are expected to be more effective, quality, quality maintained with controlled costs. Clinical Pathway as a risk evaluation mechanism evaluation tool to detect active and latent errors (latent / system errors) and near misses in Clinical Risk Management in order to maintain and improve patient safety. The purpose of writing this systematic review is to see the effectiveness of the application of clinical pathways related to risk management to service quality.Method: The search was carried out using a data base in writing a systematic review namely; Pubmed, Proquest, Willey and secondary searching, journals restricted to the last 15 years and other publications related to the topic of writing. The journals obtained used surveys and statistical descriptions to see the effectiveness of the application of clinical pathways to the quality of services in hospitals.Results: Overall the results of several research surveys of the 7 articles reviewed showed a significant positive correlation between the application of clinical pathways and the reduction in clinical risk of patient care in hospitals. There has been a stratification of the reduction in the number of days of hospitalization of patients and the index of hospitalization costs of patients in the hospital on the application of clinical flow so that it explicitly prevents over procedures such as the number of laboratory tests, number of consultations and medicines.Conclusion: The development of clinical pathway is proven to be able to improve multidisciplinary intra and inter communication, good teamwork and efficient maintenance planning. It also will automatically improve patient output, improve patient safety and provide patient satisfaction regarding service quality. To maintain optimal results and prevent the possibility of the impact of risks in clinical procedures, it is necessary to have intense, ongoing monitoring and commitment from the hospital and health workers in the implementation of clinical pathways in hospitals.

Abstract

Introduction: The "Patient safety" movement has become a spirit in hospital nursing services throughout the world, not only hospitals in developed countries that implement patient safety to ensure the quality of health and nursing services, but also hospitals in developing countries, such as Indonesia.Objective: The relationship between the implementation of the functions of planning, organizing, directing and controlling the head of the room with the application of patient safety by nurses in Makassar City Hospital.Method: this research is a quantitative research with cross sectional approach. The study was conducted in the inpatient room of the Makassar City General Hospital of 63 nurses implementing it (simple random sampling). The method of collecting data using a checklist questionnaire by distributing questionnaires, the implementation of head room management functions with the application of patient safety using the Spearman 's rho correlation test.Results: The results showed that there was a significant relationship between the function of head room management with the application of patient safety by nurses in Makassar City Hospital with a value of p = 0.009 and a Spearman's rho correlation value of 0.327, in organizing a value of p = 0.042 and a spearman correlation value of 0.257, at controlling for p value = 0,023 and spearman correlation value 0,285. There is no significant relationship between the implementation of the head room planning management function obtained p value = 0.095 and Spearman's correlation value rho 0.212, the head room directional function with the application of patient safety with a value of p = 0.530 and the spearman correlation value of 0.081. Discussion : of the results showed that the more the better the implementation of the head room management function, the better the implementation of patient safety by nurses in Makassar City HospitalConclusion: The head of the room needs to always improve the planning and direction function in an effort to civilize the application of patient safety so that there will be a good quality of patient safety in the Makassar City Hospital service environment.Keywords: Head of Management Function room, patient safety, nurses
