Abstract

Prevalensi asma yang terus meningkat menjadi perhatian bagi tenaga kesehatan. Kepatuhan pasien asma dalam penggunaan obat dan terwujudnya kualitas hidup pasien yang baik menjadi tujuan utama dalam penatalaksanaan asma. Penelitian ini bertujuan untuk mengetahui hubungan antara kepatuhan penggunaan antiasma dengan kualitas hidup pasien asma. Populasi target dalam penelitian ini adalah semua pasien rawat jalan yang  menggunakan terapi anti asma pada tahun 2012. Sampel penelitian ini adalah pasien asma dewasa dengan kategori asma menetap. Sebanyak 72 pasien diambil secara purposive dari populasi target. Penelitian ini menggunakan metode penelitian non eksperimental dengan rancangan cross sectional. Pengambilan data dilakukan secara concurrent dan retrospektif. Tingkat kepatuhan pasien diukur menggunakan kuesioner MMAS (Morisky Medication Adherence Scale) dan kualitas hidup pasien diukur menggunakan AQLQ (Asthma Quality of LifeQuestionnaire). Hasil penelitian menunjukkan 52,78% pasien memiliki kepatuhan rendah dan sebesar 47,22% pasien memiliki kepatuhan sedang. Faktor yang berhubungan secara signifikan dengan kepatuhan adalah pola kunjungan pasien. Jumlah pasien asma dengan kualitas hidup kurang baik sebesar 30,56% dan pasien dengan kualitas hidup sedang sebesar 69,44%. Penelitian menyimpulkan bahwa tidak terdapat hubungan antara kepatuhan penggunaan antiasma dengan kualitas hidup pasien asma (p = 0,842).

Abstract

Type 2 diabetes mellitus (T2DM) is a chronic metabolic disease that requires lifelong therapy to control blood glucose. The long term use and different mechanism of action antidiabetic have the potential cause side effects and discomfort that affects the quality of life. This study aimed to determine differences in the quality of life of patients with T2DM based on antidiabetic prescribed and complications at the Outpatient of Panembahan Senopati Bantul Yogyakarta Hospital in September 2017. This research is observational with a cross-sectional design. Quality of life data is taking concurrently using the Diabetes Quality of Life Clinical Trial Questionnaire and patient's medical record. This study involved 200 T2DM patients who received antidiabetic at least 3 months before study, aged ?18 years and agreed to inform concent. Sociodemographic data, antidiabetic prescribed and complications were described descriptively. Differences in the quality of life based on antidiabetic prescribed and complications using the Kruskal Wallis test which was further analyzed using the post hoc by Mann Whitney test. The results showed the majority of patients received oral combination with insulin (48.5%) and dominated by macrovascular complications (19.5%). The average value of patient's quality of life is 65.7. There was difference quality of life based on antidiabetic prescribed in the domain of treatment satisfaction (p=0.000) between oral monotherapy with combination of oral and insulin (p=0.000) and insulin monotherapy with combination of oral and insulin (p=0.002). There were differences in quality of life based on complications in the mental health domain (p=0.003) between macrovascular and microvascular (p=0.011) and between microvascular and without complications (p=0.001).

Abstract

Balanced Scorecard performance measurement (financial perspective, customer perspective, internal business perspective, and learning and growth perspective) offers a more thorough and comprehensive evaluation in a public organization such as a hospital. The object of this study was the Department of Pharmacy of RSUD X. This study was descriptive non-experimental. The data were obtained retrospectively and prospectively. The qualitative data were obtained through questionnaires. The quantitative data were obtained through direct observation, surveys. From the research using the Balanced Scorecard data gathering, it has been found that: performance customer perspective showed that the average patient satisfaction was 88% of patients. They were satisfied with the service of the Department of Pharmacy, and the average growth of customers has increased by 10% each month. In conclusions, the working performance of the department of pharmacy was included in a good criteria, with some suggestions and improvements that need to be taken into consideration.

Abstract

Counseling is one part of the clinical pharmacy service at the pharmacy that is the responsibility of the pharmacist to achieve therapeutic goal. Community needs for counseling have increased so that the active participation of pharmacists is needed through the provision of professional services. This research needs to be done to find out the amount of counseling services by pharmacists who are willing to be paid by patients with a WTP (Willingness to Pay) value parameter. In addition, the value of WTP can also describe the indirect benefits felt by the community towards counseling services by pharmacists at the pharmacy. This research is a descriptive observational research through a survey. The study was conducted at the pharmacy in the Polokarto sub-district of Sukoharjo regency in January – February 2016 with pharmacy visitor respondents who received counseling services by pharmacists. A total of 82 patients participated in this study. The amount of WTP is determined based on the average WTP value chosen by the patients with the payment card method. The result showed that the average WTP value of pharmacist counseling services at pharmacies in Sukoharjo regency was Rp. 15,892.

Abstract

ABSTRACTDiabetes Mellitus is a chronicle disease that gives effect to patient well being. Moreover, a complication suffering gives negative effect to the patient. The aim of this research is to identify the life quality of Diabetes Mellitus (type 2) sufferer and the characteristic relating to it quality of life. This research applied descriptive design and performed on the visitors of Policlinic of the interior disease on Tidar Hospital of Magelang during Juli – August 2007. The inclusion criterion is the visitors when this research performed, entering 18 years old or more, of Diabetes Mellitus Type 2, any agreement of informed consent and filled up the questionnaire. The sample taking is done by accidental sampling. To measure the life quality is used a questionnaire of Diabetes Quality of Life (DQOL), then it continued by measure the quality of life of any patients that involved therapeutic satisfaction, the effect of therapy, the anxiety of diabetes, the anxiety of social problem and the quality of life in general. Every question is measured by the Likert Scale of 5. Of the 50 analyzed patients, 46% is female and 54% is male, with the biggest rate of age is 60-69 years old.The treatment of DM type 2 in Tidar Magelang general hospital for 50 patients who do treatment in the house have good rate life of score. It is 0,599 (it means the quality of life is higher). The lowest score of therapeutic effect is 0,588. Moreover, the highest score of anxiety to the diabetes is 0,616. The male patients and the patient with long duration show the low quality of life. Accordance with the correlation test of Spearman and Kendall is obtained that there was no significant correlation between the patient characteristic and it quality of life. Abstract

ABSTRACTThe research about rationality of using over the counter drugs common cold for self medication in district of Gajahmungkur Semarang has been carried out. Data gotten from the result of field survey which is using the questioners that has been filled by Gajahmungkur’s people. Data will be analyzed with non analytical descriptive method. Based on the survey result, knowing that people in Gajahmungkur’s district choosing OTC drugs especially analgetic, antihistamin and decongestan (66,4%). The evaluation of rationality of using drugs for common cold has result: 1) Drugs of choice for common cold is correct. 2) Correct patient are 92,1%. 3) Correct dosage form 96,0%. 4) The most adverse drugs happens is sleepy (53,4%). Abstract

ASTRACTDiabetes melitus is metabolic disease characterized hiperglikemia that caused disparity of insulin secretion, insulin activity or both. WHO had predicted there were increasing of diabetes mellitus patients in next time, included in Indonesia. This study aim to know and evaluate the rational of in patients diabetes mellitus type 2 medication in Bhakti Wira Tamtama Hospital Semarang at 2006. This study is non experimental research that analyzed by non analytical descriptive method. Data were collected retrospectively from medical record comprised patient identity, diabetes melitus type 2 medication, and rational antidibetic usage. Data obtained were analysed with Standard of Perkeni 2006. The result of this study showed that 34 in-patient diabetes mellitus type 2 in Bhakti Wira Tamtama Hospital Semarang at 2006 were 40-50 years old (47,06%). Diabetes mellitus type 2 patients comprised 18 men and 16 women, 8 cases (23,52 %) were diabetes mellitus type 2 without additional diseases and 26 cases (76,47%) diabetes mellitus type 2 with additional diseases. Patients had diagnosis appropriate were 94,12 %, drug choise appropriate were 100 %, which biguanid metformin usage were 15 patients (46,87%). Dosis appropiate 100%, patient appropriate 100%, while drug interaction had happened to 1 case, that was antidiabetic and thiazid diuretic. Abstract

Indonesian Case Based Groups (INA-CBGs) implementation on hospital financing for stroke patient using Jaminan Kesehatan Masyarakat (Jamkesmas) at 3rd class hospitalization requires therapy plan management and cost analysis because stroke is a leading cause of death worldwide and need high treatment cost. It is purposed to skimp the hospital expenditure and avoid deficit suffering. The study objectives was to determine the suitability cost between real stroke treatment cost and health financing based on INA-CBGs, and to determine the highest cost component on stroke treatment at Jogja hospital. This cross sectional observational study used retrospective sampling method. The subjects were taken from all stroke patient population using Jamkesmas insurance at Jogja Hospital, were hospitalized between January 2011-April 2012 and met the inclusion criteria. The subject was then classified based on INA-CBGs. Patient?s length of stay and costs (direct medical and direct non-medical costs) were defined as study variable. Data was analysed using descriptive analysis and Mann Whitney test. Throuh this study we conclude the average rate of real stroke treatment cost at Jogja Hospital was higher insignificantly than INA-CBGs based cost and the highest treatment cost was on the medicine and medical equipment.

Abstract

Diabetes melitus merupakan penyakit kronis yang membutuhkan banyak biaya, maka analisis biaya menjadi hal yang penting untuk dipertimbangkan. Tujuan penelitian ini untuk mengetahui komponen biaya, rata-rata biaya keseluruhan, dan mengetahui apakah terdapat korelasi antara faktor pasien dan jenis obat terhadap biaya total, antara jenis pembiayaan dan kelas perawatan terhadap biaya medis langsung pada pasien diabetes melitus rawat inap di RSUD Dr. Moewardi Surakarta. Penelitian menggunakan rancangan deskriptif non eksperimental yang berasal dari pasien kelompok 1 (JAMKESMAS + PKMS + tidak mampu) dan kelompok 2 (ASKES PNS + Umum + Kerjasama), data diambil secara retrospektif dari catatan medik pasien dengan diagnosis utama diabetes melitus dengan penyakit penyerta yang memenuhi kriteria inklusi selama periode Januari sampai dengan Desember 2010, catatan bagian keuangan RSUD Dr. Moewardi Surakarta untuk mengetahui biaya serta rincian penggunaan obat dari bagian instalasi farmasi. Pengambilan sampel menggunakan teknik simple random sampling. Uji chisquare digunakan untuk mengetahui hubungan antara faktor pasien, jenis obat, jenis pembiayaan, dan kelas perawatan terhadap biaya total. Hasil penelitian menunjukkan bahwa komponen biaya yang menyusun keseluruhan biaya adalah biaya medis langsung. Rata-rata biaya pengobatan total pada pasien kelompok 1 adalah Rp 4.127.180 dan pada pasien kelompok 2 adalah Rp 3.828.282. Komponen biaya terbesar pada pasien kelompok 1 adalah biaya laborat (27,02%), biaya obat (25,74%), dan biaya tindakan (17,20%), sedangkan pada pasien kelompok 2 komponen biaya terbesar adalah biaya obat (27,54%), biaya laborat (23,02%), dan biaya tindakan (19,00%). Hasil uji chisquare menunjukkan ada hubungan antara lama waktu perawatan (LOS) dengan biaya pengobatan diabetes melitus di RSUD Dr. Moewardi Surakarta. Kata kunci: analisis biaya, diabetes mellitus
