Abstract

Abstrak Latar Belakang: Gejala sumbatan hidung meskipun bukan suatu gejala penyakit yang berat, tetapi dapat menurunkan kualitas hidup dan aktivitas penderita. Penyebab sumbatan hidung dapat bervariasi dari berbagai penyakit dan kelainan anatomis. Salah satu penyebab dari kelainan anatomi adalah deviasi septum nasi. Tujuan: Untuk menilai gejala dan derajat sumbatan hidung pada deviasi septum nasi. Tinjauan Pustaka: Diagnosis dari gejala sumbatan hidung sangat kompleks dan bervariasi, selain berdasarkan anamnesis dan pemeriksaan fisik juga diperlukan pemeriksaan penunjang untuk pengukuran sumbatan hidung. Skor sumbatan hidung merupakan salah satu parameter untuk menilai suatu sumbatan hidung pada deviasi septum nasi. Untuk itu diperlukan pemeriksaan penunjang yang dapat digunakan untuk mendiagnosis dan mengevaluasi gejala sumbatan hidung, diantaranya adalah nasal inspiratory flow meter, rhinomanometri dan rhinometri akustik. Kesimpulan: Gejala sumbatan hidung pada deviasi septum dapat dievaluasi dengan pemeriksaan tambahan meliputi pemeriksaan dengan spatula lidah, nasal inspiratory flow metry, nasal expiratory flow metry, rinomanometri, dan rinometri akustik. Abstract Background: Although nasal obstruction is not a severe symptom of the disease, it can decrease the quality of life and activity of the patient. The etiology of nasal obstruction could be varied from any diseases and anatomical abnormalities. One of anatomical abnormality cause is septal deviation. Purpose: To evaluate the symptom and the degree of nasal obstruction in septal deviation. Review: The diagnosis of nasal obstruction is more complex and varied, based on anamnesis and physical examination, and beside that need additional examination to measure the nasal patency. Nasal obstruction score is one of parameter to evaluate the obstruction of nose. Because of that, it needs additional examination to diagnose and evaluate the nasal obstruction, include nasal inspiratory flow meter, rhinomanometry, acoustic rhinometry. Conclusion: Nasal obstruction in septal deviation with additional examination, such as tongue spatula, nsal expiratory flow metry, nasal inspiratory flow meter, rhinomanometry, acoustic rhinometry. Abstract

AbstrakCapillary haemangioma is a benign, rapidly growing lesion of the skin and mucous membranes. It may rarely present as a mass of considerable size and thus entirely fill the nasal cavity. Its etiology remains obscure. Capillary haemangioma usually involves the gingiva, lips, tongue, and buccal mucosa. However, the nasal cavity is a rare location for this lesion. The most common symptoms are unilateral epistaxis and nasal obstruction. The treatment of choice is surgery to remove the tumour even for large lesion. A case of capillary haemangioma at anterior nasal septal in 6 years old girl had been treated with extirpation.Kata kunci: Capillary haemangioma, nasal septum, epistaxis, diagnosis, treatmentAbstractHaemangioma kapiler merupakan suatu lesi jinak pada kulit dan mukosa dengan pertumbuhan yang cepat. Meskipun jarang berukuran besar, namun dapat memenuhi seluruh kavum nasi. Etiologinya sampai sekarang masih belum jelas. Haemangioma kapiler sering terdapat pada ginggiva, bibir, lidah dan mukosa bukal. Kavum nasi merupakan lokasi yang jarang terdapatnya haemangioma kapiler. Gejala yang paling sering adalah unilateral epistaksis dan sumbatan hidung. Pembedahan untuk mengangkat tumor ini merupakan terapi pilihan meskipun berukuran besar. Suatu kasus haemangioma kapiler septum anterior pada anak perempuan berumur 6 tahun telah dilakukan ekstirpasi sebagai terapi.Keywords: Haemangioma kapiler, septum nasi, epistaksis, diagnosis, penatalaksanaan

Abstract

AbstrakLatar belakang: Rinitis alergi merupakan penyakit dengan insiden yang cukup tinggi diseluruh dunia dengan prevalensi yang semakin meningkat setiap tahun. Patogenesis rinitis alergi melibatkan reaksi imun yang cukup komplek. Tujuan: Mengetahui peranan kemotaktik sitokin (Kemokin) dalam patogenesis rinitis alergi. Tinjauan pustaka: Kemokin sebagai kemotaktik sitokin berperan dalam semua tahap reaksi alergi. CC kemokin merupakan subfamili kemokin yang berperan dalam reaksi alergi. Kemokin bekerja pada permukaan sel-sel inflamasi berikatan dengan reseptor. CCR3 merupakan reseptor dengan kadar tertinggi yang ditemukan pada permukaan eosinofil dan eotaxin sebagai ligand yang spesifik bagi CCR3. Kesimpulan: Eotaxin dan reseptor CCR3 adalah faktor yang paling menonjol dalam patogenesis rinitis alergi yang melibatkan eosinophilKata kunci: Reaksi alergi, Th2, CC Kemokin, eosinofil, CCR3 AbstractBackground: Allergic rhinitis such a disease with high incidence among the world and its prevalence appears to be increasing every year. The pathogenesis of allergic rhinitis commit complex imunological reaction. Purpose: To know the role of chemotactic cytokine (chemokine) in pathogenesis of allergic rhinitis. Literature review: Chemokine as a chemotactics cytokine participate to all allergic reaction stage. CC chemokines were subfamilial chemokines which have role in allergic reaction. They working by binding with receptor on the surface of inflamatory cells. CCR3 is the receptor with the highes level could be found on eosinophil cell membran and eotaxin was the spesific ligand to itt. Abstract

AbstrakLatar belakang: Pleomorfik adenoma parotis merupakan tumor jinak kelenjar liur yang paling sering ditemukan, namun pleomorfik adenoma parotis yang sangat besar sehingga melibatkan ruang parafaring (RPF) sangat jarang. Diagnosis ini sulit ditegakkan karena gejala klinisnya tidak khas. Penatalaksanaanya harus hati-hati mengingat banyak struktur vital yang beresiko mengalami trauma. Tujuan: Bagaimana menegakkan diagnosis dan penatalaksanaan pleomorfik adenoma parotis yang melibatkan RPF. Kasus: Seorang pasien perempuan 27 tahun ditegakkan diagnosis pleomorfik adenoma parotis kanan dengan melibatkan RPF. Terdapat pembengkakan pada leher yang bersifat asimtomatis dan gejala pendorongan faring dan laring yang menyebabkan disfonia, disfagia, dan defisit saraf kranial IX,X,XII. Penatalaksanaan: Pasien telah dilakukan operasi parotidektomi pendekatan transervikal–transparotid dengan preservasi arteri karotis eksterna dan saraf fasialis. Kesimpulan: Biopsi Aspirasi Jarum Halus (BAJAH) dan radiologi merupakan pemeriksaan yang penting untuk menegakkan diagnosis. Penatalaksanaan pleomorfik adenoma parotis yang melibatkan RPF adalah bedah ekstirpasi komplit dengan beberapa pendekatan. .Kata kunci: tumor jinak kelenjar liur, pleomorfik adenoma, ruang parafaringAbstractBackground: Parotid pleomorphic adenoma is the most common benign salivary gland tumor, while giant parotid pleomorphic adenoma involving the parapharyngeal space (PPS) is rare. It was difficult to diagnose because the clinical presentation of this tumor can be subtle. The management must be performed carefully due to anatomy relation to complex vital structure lead to traumatic injury highrisk. Purposes: How to make diagnosis and management parotid pleomorphic adenoma involving PPS. Case: A female 27 years old with diagnosis was giant parotid pleomorphic adenoma involving PPS. There was asymptomatic swelling of the neck and presence of pushing the pharynx and larynx medially causes dysphonia, dysphagia, and IX,X,XII cranial nerves deficit. Management: The patient has been performed parotidectomy with transcervical-transparotid approaches by preservation of the external carotid artery and facial nerve. Conclusion: Fine Needle Aspiration Biopsy (FNAB) and imaging are essential for diagnostic. The management of parotid pleomorphic adenoma involving PPS is surgical complete extirpation with various approaches.Keywords:benign salivary gland tumor, pleomorphic adenoma, parapharyngeal space

Abstract

AbstrakAbses septum nasi adalah terkumpulnya pus di antara tulang rawan dengan mukoperikondrium atau tulang septum dengan mukoperiosteum yang melapisinya. Abses septum dapat menyebabkan hidung pelana bahkan komplikasi intrakranial, sehingga diperlukan diagnosis dan tindakan yang tepat dan cepat. Telah dilaporkan satu kasus abses septum pada wanita umur 34 tahun dan telah dilakukan insisi dan eksplorasi abses dalam narkose umum.Kata kunci: Arial 9 Kata Kunci: Abses septum nasi, hematoma septum, hidung pelana.AbstractNasal septal abscess is defined as pus accumulation between cartilage and mucopericondrium or septal bone and mucoperiosteum which is layer it. Septal abscess can cause saddle nose even intracranial complications, requiring additional diagnosis and appropriately and quickly management. Reported one case of septal abscess in woman 34 years old and has been done the abscess incision and exploration in general anaesthesia.Keywords:Nasal septal abscess, haematoma septum, saddle nose.

Abstract

AbstrakGejala sumbatan hidung meskipun bukan suatu gejala penyakit yang berat, tetapi dapat menurunkan kualitas hidup dan aktivitas penderita. Salah satu penyebabnya adalah deviasi septum nasi. Pemeriksaan penunjang yang dapat digunakan untuk mendiagnosis dan mengevaluasi gejala sumbatan hidung, diantaranya adalah Nasal Inspiratory Peak Flowmeter (NIPF). Tujuan penelitian ini adalah untuk mengetahui pengaruh septoplasti terhadap sumbatan hidung pada deviasi septum dengan pemeriksaan NIPF. Metode penelitian ini adalah eksperimental studi dengan teknik pre dan post-test design untuk mengetahui gambaran hasil NIPF pada penderita deviasi septum nasi dengan sumbatan hidung. Pengukuran NIPF dilakukan sebelum operasi, minggu ke-2, ke-4 dan ke-6 setelah operasi. Hasil penelitian ini menunjukkan terdapat 11 pasien (93%) dari 12 pasien secara subyektif mengalami perbaikan sumbatan hidung. Terdapat perubahan sumbatan hidung yang bermakna pada minggu ke-4 (p=0,01), dan ke-6 (p=0,01). Kesimpulan penelitian ini adalah septoplasti dapat memperbaiki sumbatan hidung pada deviasi septum nasi.AbstractEven though nasal congestion is not a severe symptom, it can reduce quality of life and patient’s activities. One of the causes of nasal congestion is septal deviation. Diagnostic test that could be used to evaluate nasal congestion is Nasal Inspiratory Peak Flowmeter (NIPF). The objective of this study was to measure the effect of septoplasty to nasal congestion caused by septal deviation with NIPF examination. This research was experimental study by pre and post-test design to evaluate NIPF of patients with nasal congestion due to septal deviation. NIPF was measured before operation surgery, second week, fourth week and sixth week after surgery. The result showed that there were 11 patients (93%) of 12 patients with decreased of nasal congestion subjectively. There were significant decrease of nasal congestion at fourth week (p=0.01) and sixth week (p=0.01). The conclusion of this study is septoplasty can reduce nasal congestion on septal deviation.

Abstract

Introduction: Patients with nasopharyngeal carcinoma have a poor prognosis, there are several factors that cause it to happen, one of the existing therapeutic response has been inadequate. Expression of Epidermal Growth Factor Receptor (EGFR) has been used as a biological marker targeted therapy in nasopharyngeal carcinoma. Histopathologic subtype tumors also determine the prognosis of patients with nasopharyngeal carcinoma. Objective: The aim of the study to determine between the expression of epidermal growth factor receptor between non-keratinized differentiated and undifferentiated subtypes in nasopharyngeal carcinoma and correlation with their clinical stage. Study design, Cross-sectional comparative study. Place and duration study, Department of Otorhinolaryngology, Department of Pathology Anatomy in Dr. M. Djamil Hospital, Padang and Department of Pathology Anatomy in Gajah Mada University, between May 2015 until October 2015 Material and methods: We included 36 samples paraffin blocks of nasopharyngeal carcinoma biopsy, respectively 18 paraffin blocks are non-keratinized differentiated and 18 non-keratinized undifferentiated nasopharyngeal carcinoma subtypes. Each sample examined EGFR expression by immunohistochemical staining methods. Results: There were positive EGFR expression results in all sample as 69.4%. Expression of EGFR positive non-keratinized differentiated subtypes in nasopharyngeal carcinoma as 77.8% and undifferentiated subtype as 61.6%. There are no significant differences of EGFR expression between non keratinized differentiated and undifferentiated subtypes nasopharyngeal carcinoma (P>0.05). There are no significant differences of EGFR expression between new and advanced stage nasopharyngeal carcinoma (P>0.05). Conclusion: There were no significant differences of EGFR expression between non-keratinized differentiated and non-keratinized undifferentiated subtypes in nasopharyngeal carcinoma. Analysis of the study also showed no significant differences of EGFR expression based on the clinical stage nasopharyngeal carcinoma.
