Abstract

Epidemiologically, the percentage of colorectal adenocarcinoma (CRC) in the age group ≤40 years old in Indonesia is higher than in the West European, North American and other developed Asian countries with different clinico-pathological features. In the afromentioned countries, the carcinogenesis pathway of this group is hereditary which show high microsatellite instability (MSI), whereas the age group >40 years old is sporadic of which show chromosomal instability (CIN) with the mutation of p53 gene. To elucidate the carcinogenesis pathway with its molecular characteristics of this group among Indonesians, a cross sectional study was conducted by performing the immunohistochemical detection of MLH1 and MSH2 expression (MSI status), and mutated p53 gene expression (CIN) in the CRC group ≤40 years old and >40 years old at Dr. Hasan Sadikin Hospital, Bandung, from May 2008 until November 2009. Thirty nine CRC patients, consisting of 22 patients of  ≤40 years old and 17patients >40 years old, were eligible for this study. The CRC patients of ≤40 years old showed 4 MSI high,  1 MSI lowand 17 MSS (microsatellite stable), associated with 10 p53 positive and 12 p53 negative status. In the CRC group of >40 years old, there was no MSI high, but 4 MSI low and 13 MSS were found, associated with 11 p53positive and 6 p53negative status. There was no significant difference with regards to the association between MSI status and p53 expression in both groups (p MSI=0.95, p p53=0.23).

Abstract

Background: Regular physical activity is highly recommended in preventive, curative, and rehabilitative programs in order to promote health, especially cardiovascular health. However, physical activity can also cause sudden death. In athletes, sudden death may occur during sport competitions, with myocardial infarction as the most common etiology. It is suspected that continuous training without any rest-day play a role in cardiac muscle damage and sudden death during competition. Our study was aimed to learn about cardiac muscle adaptation on continuous aerobic and anaerobic physical activity without any rest-day. Methods: The specimens in our study were cardiac muscle tissue obtained from rats that had performed aerobic and anaerobic physical activity on treadmill for 1, 3, 7, and 10 days without any rest-day. Blood gas analysis and hematological assessment were used as parameters of systemic adaptation to hypoxia during physical activity. Moreover, histopathology of cardiac muscle tissue was performed as parameter for cardiac muscle damage.Results: The results showed that aerobic and anaerobic physical activity caused a systemic hypoxic condition and triggered adaptation responses. Cardiac muscle damage occurred on the 10th day in both treatment groups, with more severe damage observed in the group with anaerobic physical activity. The tissue protein level in the anaerobic group increased progressively on the 10th day.Conclusion: Physical activity may result in hypoxia and systemic adaptation. Aerobic and anaerobic physical activities performed for 10 days without any rest-day may cause cardiac muscle damage. Abstract

Vascular endothelial growth factor (VEGF) merupakan faktor angiogenik yang berperan dalam angiogenesis tumor. Tissue factor (TF) merupakan inisiator utama pembekuan darah dan merangsang protein yang mengatur angiogenesis. Penelitian bertujuan mengetahui perbedaan ekspresi VEGF dengan ekspresi TF pada penderita KNF stadium lanjut berdasarkan respons terapi kemoradiasi. Dilakukan penelitian kohort prospektif pada penderita KNF stadium III, IVa, dan IVb berdasarkan AJCC edisi ke-7 tahun 2010. Ekspresi TF dan VEGF diperiksa dengan imunohistokimia dan respons kemoradioterapi dievaluasi dengan memakai Response Evaluation Criteria in Solid Tumours (RECIST) revisi versi 1.1 tahun 2009. Analisis statistik yang digunakan adalah Uji eksak Fisher. Selama penelitian Oktober 2012?Oktober 2013 didapatkan 35 penderita KNF yang memenuhi kriteria inklusi dan 5 orang dikeluarkan dari penelitian. Pada kelompok respons didapatkan 17 dari 23 penderita ekspresi VEGF ? 25%; 6 dari 23 penderita ekspresi VEGF <25%; 16 dari 23 penderita ekspresi TF ? 33%; 7 dari 23 penderita ekspresi TF <33% (p=1,000) serta pada kelompok tidak respons 5 dari 27 penderita ekspresi VEGF ?25%; 2 dari 7 penderita ekspresi VEGF <25%; 4 dari 7 penderita ekspresi TF ? 33%; 3 dari 7 penderita ekspresi TF <33% (p=0,657). Simpulan, tidak terdapat perbedaan ekspresi VEGF dengan ekspresi TF pada penderita KNF stadium lanjut berdasarkan respons terapi kemoradiasi. Tissue factor (TF), the main initiator of blood coagulation, also signals protein that regulates angiogenesis. This study  analyzed the differences between VEGF expression and TF expression in tumor tissue based on chemoradiation therapy response. Prospective cohort study was performed in NPC patients stage III, IVa and IVb according to the AJCC VII staging system. TF expression and VEGF expression were measured by immunohistochemistry, and chemoradiotherapy responses was evaluated by RECIST version 1.1 2009. Statistical analysis was performed using Fisher Exact test. From October 2012 to October 2013, 35 NPC patients were eligible for this study and 5 patients were excluded. In response group, there were 73.9% patients with VEGF expression ?25%, 26.1% patients with VEGF expression <25%; 69.6% patients with TF expression ?33%, 30.4% patients with TF expression <33% (p=1,000) and in no response group, there were 71,4% patients with VEGF expression ? 25%; 28.6% patients with VEGF expression <25%, 57.1% patients with TF expression  ?33%, 42.9% patients with TF expression <33% (p=0.657). In conclusion, there are differences between VEGF expression and TF expression based on the chemoradiation therapy response, but they are not significantly different. Abstract

Histopathology grade of meningioma is one of the most common factors determining the prognosis. That affects the risk of recurrence and aggressiveness of the tumor. Biological factors related to histopathological grade are vascular endothelial growth factor (VEGF-A) and programmed death-ligand1 (PD-L1). This research aimed to understand the association between VEGF A and PD-L1 with meningioma histopathology grade. This is In-vivo research on 60 paraffin blocks of meningioma cases at the Hasan Sadikin Hospital, Bandung. Paraffin block samples consist of grade I (30), grade II (15), and grade III (15) meningioma. Immunohistochemical (IHC) staining of VEGF-A and PD-L1 performed to all samples. Data analyzed using the chi-square test with SPSS 24.0 for Windows. The result shows a statistically significant association between VEGF-A and PD-L1 immunoexpression with meningioma histopathology grade . PD-L1 is the most potent factor that influenced the histopathology grade of meningioma. The study concluded that the histopathology grade of meningiomas influenced by angiogenesis and immune checkpoints. Abstract

Background: Breast cancer is the most common type of cancer that has been diagnosed among Indonesian women. One of the subtypes that frequently being studied is the triple negative breast cancer which has poorer prognosis among all subtypes. The objective of this study was to investigate the incidents and the characteristics of triple negative breast cancer patients.Methods: This study used triple negative breast cancer patients’ medical records in Dr. Hasan Sadikin General Hospital. Some characteristics were identified, such as age at presentation, menopause status, tumor size, stage at presentation, grades of tumor, pathological features, and metastatic status.  Data were taken during 2010 and 2011 and presented in table.Results: Thirty women among 252 patients were identified as having triple negative breast cancer. Based on those 30 cases, 11 patients were in the age group of 40‒55 years. Fifteen out of 30 cases were premenopausal. From 30 cases, 14 patients had tumor size bigger than 5 cm.  Sixteen cases had stage III tumor.  Most of the cases were invasive ductal carcinoma mammae as their histological features. Two cases had metastized to pleura.Conclusions: The incidence rate of triple negative breast cancer in Dr. Hasan Sadikin General Hospital is 11.9% with these common characteristics: range of age are between 40 to 45 years, have a premenopausal, had >5cm tumor , stage III tumor and has histological feature IDC, and only 2 patients were metastized to pleura. DOI: 10.15850/amj.v2n3.494

Abstract

Nasopharyngeal carcinoma (NPC) is an epithelial malignancy originating from the surface of the lateral and posterior wall of the nasopharynx. Bcl-2 is an oncoprotein that plays an important role in disrupting the process of apoptosis. Expression of Bcl-2 in biopsy samples of undifferentiated NPC also related to the nature of more aggressive tumor mass and unfavorable clinical radiotherapy response. The purpose of this study was to assess the relationship of Bcl-2 expression with radiotherapy response in undifferentiated NPC. The method of this study areusing 40 samples of paraffin blocks were diagnosed as undifferentiated NPC by histopathologic examination with H&amp;E staining. The samples were divided into 2 groups who respond and do not respond to radiotherapy. All samples examined by Bcl-2 immunohistochemistry and the expression willanalyzed and assess therelationship to radiotherapy response. Of the 40 cases of undifferentiated NPC show imunoekspresi Bcl-2 positive in 39 cases (97.5%) and 1 case (2.5%) negative from response group. The results of statistical analysis of the Bcl-2 obtained significant results with p value &lt;0.05 so we can conclude there is a relationship between Bcl-2 expression with radiotherapy response. Conclusion, Bcl-2 can be used as a predictor for the success of radiotherapy in undifferentiated NPC. Keywords: Bcl-2, undifferentiated NPC, radiotherapy response

Abstract

Lymphadenopathy is enlarged lymph nodes caused by infection, inflammation or malignancy. On HIV positive patients, lymphadenopathy is one of the most common clinical manifestations and it is usually persistent. Fine-needle aspiration biopsy (FNAB) is an effective cytology technique in determining the diagnosis of lymphadenopathy. This study aimed to describe the cytopathology of lymphadenopathy in HIV positive patients. This is a descriptive study of 21 cases of lymphadenopathy in patients with HIV positive who underwent FNAB examination in Anatomical Pathology Department of Dr.Hasan Sadikin Hospital between 2013-2014. Medical data was taken from the patient medical records including age, sex, location, size and cytopathological diagnosis. Cytopathology overview of FNAB specimens were reassessed by 2 pathologists. In this study, lymphadenopathy in HIV positive patients were mainly found in men (n = 15.71%) with an average age between 20-30 years. The most frequent location was the neck (n = 20.95.2%). The lymph nodes size were found between 0.5-3 cm. Most diagnosis was tuberculous lymphadenitis (n = 15.71%) with the most common cytology feature was granulomatous lymphadenitis (n = 5.33.3%) and suppurative lymphadenitis (n = 5.33.3%). FNAB examination in lymphadenopathy is very helpful to identify the cause of infection in HIV positive patients. Keywords: FNAB, HIV, lymphadenopathy, cytopathology

Abstract

Wound examination is indispensable in forensic practice. The scientific field of wound age determination has advanced progressively during recent years.The purpose of this study was to determine the differences of fibronectin and TGF-β1 expression in both antemortem and postmortem wounds. This study was an experimental with completely randomized design.  The skin wounds (vital and postmortem) were taken from fourty Wistar rats and divided into 10 groups of rats. Immunohistochemical staining was performed to determine the differences between antemortem and postmortem wounds. The result showed that in 30 minutes after antemortem wound infliction, all of samples showed weak reactivity for fibronectin and TGF-β1 (100%).  In first hour after wound infliction, 3 samples (75%) showed weakly positive and 1 sample (25%) strongly positive for fibronectin and TGF-β1.  In 2 hour after wound infliction, 1 sample (25%) showed weakly positive and 3 sample (75%) strongly positive for fibronectin and TGF-β1.  In 3 and 4 hour after wound infliction, all of samples strongly positive for fibronectin and TGF-β1.  In postmortem wound, all of samples showed negativity for fibronectin and TGF-β1. In conclusion, fibronectin and TGF-β1 may be useful in the determination of wound vitality. Keywords: wound, fibronectin, TGF-β1, vitality

Abstract

Background: Carcinoma is the most common tumor in nasopharynx. Endoscopy is used to determine the presence of lesions suspected of malignancy. Narrow Band Imaging is an endoscopic technique that uses narrow–band spectrum as a filter to determine any mucosal vascular changes in carcinoma. Narrow Band Imaging can early detect superficial mucosal lesions that are difficult to detect with conventional endoscopy, so that diagnosing is more accurate and occurrence of unnecessary biopsies can be reduced. This study aimed to determine diagnostic value of Narrow Band Imaging in diagnosing nasopharyngeal carcinoma.Methods: This diagnostic test study was conducted at Department of Otorhinolaryngology-Head and Neck Surgery Dr. Hasan Sadikin General Hospital, Bandung from September to October 2014. Twenty four patients with clinical signs and symptoms of nasopharyngeal carcinoma that fulfilled the study criteria were assigned using consecutive sampling to examine with Narrow Band Imaging and histopathology examination. The data were then analyzed with 2x2 table to determine sensitivity, specificity, positive predictive value, negative predictive value, and accuracy.Results: In this study, the results of sensitivity, specificity, positive predictive value, negative predictive value, and accuracy in diagnosing nasopharyngeal carcinoma using Narrow Band Imaging were 93.75%, 62.5%, 83.3%, 83.3%, and 83.3%, respectively. Abstract

Distosia bahu merupakan salah satu penyebab paling bermasalah dalam kebidanan yang berkaitan erat dengan morbiditas bahkan mortalitas janin. Tenaga kesehatan harus siap dalam  menangani kedaruratan obstetrik ini. Tenaga bidan yang berkualitas dihasilkan oleh institusi pendidikan kebidanan yang dikelola dengan memperhatikan perkembangan ilmu  pengetahuan, teknologi informasi dan regulasi, salah satunya adalah pembelajaran praktikum selama pendidikan menggunakan media video. Kegiatan praktikum  menggunakan metode dan media pembelajaran yang tepat dan bervariasi akan membawa mahasiswa kepada pembentukan keterampilan dan kreativitas dalam menerima pengetahuan. Metode penelitian adalah quasi eksperimental dengan rancangan control group design. Subjek penelitian adalah mahasiswa semester IV Prodi DIII kebidanan Universitas Ubudiyah Indonesia yang berjumlah 32 orang.Teknik pengambilan sampel menggunakan populasi seluruhnya. Analisis digunakan analisis univariat, bivariat dengan uji Mann Whitneydan t test dan multivariatmenggunakan uji manova dengan taraf signifikan 0,05. Hasil penelitian menunjukkan peningkatan pengetahuan pada kelompok perlakuan dari 76,7 menjadi 86,7, sedangkan pada kelompok kontrol 66,7 menjadi 80 yang berbeda signifikan (p=0,001).  Peningkatan keterampilan pada kelompok perlakuan 50,8 menjadi 90, pada kelompok kontrol 48,6 menjadi 75,5,  perbedaan ini secara statistik bermakna (p<0,001). Penggunaan media video berpengaruh secara bersamaan terhadap pengetahuan dan keterampilan pada kelompok perlakuan (p<0,001). Simpulan hasil penelitian, pengetahuan dan keterampilan mahasiswa tentang distosia bahu lebih tinggi pada kelompok yang menggunakan media video dibandingkan dengan kelompok tanpa media video.
