Abstract

Nusantara Health Program is a program that potentially solve the problems of distribution health workers in Indonesia. The Ministry of Healthhas targets in 2015 will deliver about 960 health workers were divided into two periods. The health worker from Nusantara Sehat team will occupied 120 health centers in the area of DTPK and DBK. In the first and second period of  implementationin 2015, the number of health workers from Nusantara Sehat team in placement,  do not meet the targets, there was only 694 health workers from 950 health workers has planned. This problem causedamount of health centers has minimal number of medical personnel. This problem is about the fulfillment of human resources in health personnel resources that will be placed as Nusantara Sehat Team. The purpose of this study is to describe how the recruitment and selection process overview Health Worker of Nusantara Sehat team, a Program from Ministry of Health of the Republic of Indonesia. This is a qualitative research with in-depth interviews. Interviews were conducted to the informant directly related to the recruitment and selection process of Nusantara Sehat program, there are Pusrengun BPPSDMK in health ministry of health of the Republic of Indonesia and Nusantara Sehat Program applicants. The results showed thatthe publication forming of recruitment has been using way where everyone can knows and the selection process conducted by two stages with knockout system. Recruitment and selection process has some obstacles, there still a lack of both doctors and dentists. This obstacle happened because the content of publication recruitment can not make applicants interested to apply.

Abstract

Infant mortality is infants death who aged 0 to 11 months. Infant Mortality Rate Purworejo from year to year has increased (in 2013 amounted to 11.54/1000 kh and 2014 amounted to 12.55/1000 kh), and still higher than IMR Central Java (10.41/1,000 kh). Region of Ngombol?s PHC is one areas that experienced high infant mortality and increased for 2 years. This study aims to determine direct and indirect causes of infant mortality in area of Ngombol?s PHC. Research method using qualitative descriptive which retrospective. Research sample are main informant consisted of 11 women whose babies died in 2014, and informant triangulation head of PHC, midwife coordinator, and 3 village midwife. Research variables consisted of maternal factors, infant, environmental, and health services. Direct causes of infant mortality in Ngombol region on 2014 was LBW, asphyxia and congenital abnormalities. Age of mother at risk (> 30 years) was found in maternal LBW infants and asphyxia. Almost all women had experience pregnancy complications such as hypertension, HB less, KEK. All mothers get exposure chemical smoke from her husband. Almost all informants have cage attached to house. Quality of antenatal, childbirth and neonate care still less than optimal, both in terms service providers and community. Most mothers do not know pregnant women class program because less socialization. GSIB program is not running as a cross-sectoral support is lacking. Conclusion this study is indirect causal factor of infant mortality in Ngombol region dominated by factors maternal age, pregnancy complications, environment exposure, and health services quality.

Abstract

Tuberculosis in Indonesia?s Correctional Institution  is not a new case. Semarang Class I Correctional Institution on March 2018 has an excess capacity of 114%, and this condition causes a high risk of TB disease. In 2016 until early 2018 has found 25 cases of TB in this Correctional Institutions, although they have been using DOTS strategy. The success of TB program is influenced by multi factors such as the involvement of the stakeholders. This study aims to mapping the position of stakeholders of TB control in Semarang Class I Correctional Institution. This type of research is qualitative with in-depth interviews. The main informants are Doctor, Monev Officer, and Cadres. The Triangulation informants are TB Wasor, Kasi Perawatan, Head of TB Team, and Prisoner as program targets. Stakeholder positions are reviewed based on attitude, influence and interest. The results shows that all stakeholders support the TB control program in Semarang Class I Correctional Institution and all stakeholders except  prisoner who have pasive interest in the program. The influence of stakeholders in the program greatly influences its strategic position, particularly its ability to influence through advocacy and supervision. From all stakeholders, only the Wasor, Kasi Perawatan, and Head of TB Team who have strong influence. The conclusion of this study is the position of Wasor, Kasi Perawatan, and Head of TB Team has a supportive attitude, strong influence and active interest in the position of the "Saviour" program. Doctor, Monitoring and Evaluation Officer and Cadres has  a supportive attitude, has a weak influence and active interest tend to be "friends". Prisoner as program target has supportive attitude, involved passively and has a weak influence on the position of "Acquintance".

Abstract

Outbreak Response Immunization (ORI) is an activity carried out as an effort to overcome the outbreak of Diphtheria. Mijen Primary Health Center is one of the health centers in Semarang that organizes ORI activities because the discovery of a positive diphtheria case. Mijen Primary Health Center has decreased coverage in the second stage of ORI with a gap of 8.3%. The research objective is to evaluate the implementation of ORI activities in Mijen District by assessing the variables of surveillance, diagnosis, clinical management, supervision and assessment, infection control, and reporting. The research method uses qualitative research with a descriptive approach. This study interviewed 10 informants. The results showed that the implementation of ORI was able to reduce the rate of transmission and death from diphtheria. But the implementation of ORI still leaves a risk factor due to a decrease in ORI coverage which will result in an immunity gap. In addition, there is a large gap in setting targets for activities that are only carried out in an area of 3 RT in 3 villages, while according to the technical guidelines for the implementation of ORI, ORI should be at least one villages and one sub-district where cases are found. Target reduction due to limited resources and vaccine availability and vaccine logistics. Decrease in coverage is influenced by there are still people who do not believe that there is a case of diphtheria because the suspect claimed to be negative diphtheria. Counseling is also not maximal because most of the participants are cadres. Suggestions can be given to the Mijen health center is providing clear information to the public regarding the presence of diphtheria cases, increasing the intensity of counseling, reviewing the ORI target setting, Using PPE while working, and providing timely reports to the City Health Office.

Abstract

Ketersediaan sumber daya manusia yang sesuai kebutuhan dengan kemampuan dan kualitas, serta professional merupakan indikator keberhasilan farmasi yang efektif dan efisien. Kecukupan tenaga masih kurang dikarenakan peningkatan jumlah resep namun tidak diimbangi jumlah tenaga sehingga meningkatkan beban kerja dan komplain pasien terkait lamanya waktu tunggu obat. Penelitian bertujuan menganalisis kesesuaian beban kerja dengan jumlah SDM dan mengetahui kendala pemenuhan jumlah tenaga.Penelitian dilakukan dengan metode deskriptif kualitatif melalui wawancara dan observasi langsung. Informan utama adalah apoteker dan tenaga teknis kefarmasian. Informan triangulasi adalah kepala seksi penunjang medis,kepala bidang kepegawaian dan direktur rumah sakit.  Analisis data menggunakan rumus WISN berdasarkan data selama observasi, melakukan kategorisasi data, melakukan verifikasi dan menarik kesimpulan.Hasil penelitian menunjukkan instalasi farmasi masih kekurangan SDM diketahui dari hasil perhitungan WISN dan wawancara dengan informan. Beban kerja farmasi belum sesuai karena jumlah pelayanan resep tidak diimbangi dengan jumlah tenaga, SPO pola dan distribusi ketenagaan belum dijalankan dengan baik dan jumlah sarana prasarana belum mencukupi. Pemenuhan tenaga sesuai beban kerja masih memiliki kendala disebabkan efisiensi biaya dan melakukan modifikasi tenaga untuk mengatasi beban kerja yang ada.Penelitian ini merekomendasikan penambahan tenaga disesuaikan beban kerja, menjalankan SPO dengan baik dan melengkapi sarana prasarana untuk meningkatkan pelayanan farmasi.

Abstract

Maternal and child health was a priority of health program in Indonesia. Maternal Mortality Rate in Semarang was ranked second highest in Central Java. The highest proportion of maternal deaths occurred during puerperium. That?s indicates, there was a problem in a process of maternal health services during puerperium period in health facilities. This study was conducted to examine constraints on health systems that limit range of interventions or health services that were important for postpartum, bottlenecks related to postpartum services in Public Health Center (PHC), especially infrastructure, human resources, access to PHC, post partum visits (KF1 and KF3), as well as quality of post partum services on risk reduction of maternal mortality. This case study used a qualitative approach. Data collection through interviews to five midwives as main informants, 5 midwives coordinator and 5 heads of PHC as informant triangulation. Data analsyis used content analysis method, then assigned priority bottleneck through MCUA (Multiple Criteria Utility Assessment) techniques. WHO's scale-up BNA plan to analyze bottleneck causes. Results showed, there was a bottleneck on childbirth services in PHC. The causes of bottleneck risk reduction efforts of maternal mortality incidence in puerperium period has never been analyzed workload of health personnel in PHC, lack of monitoring and evaluation of an availability infrastructure facilities in PHC, there has not been regular training, especially on delivery until puerperium services, and PHC has not received more detailed and operational information about puerperium so their maternal knowledge about puerperium has not increased much and couldn?t raise mother's awareness to do so. Semarang Public Health Office (Dinas Kesehatan Kota Semarang) needs to conduct periodic monitoring and evaluation implementation of postpartum services and improve quality of childbirth services in PHC.Keywords: Bottleneck analysis, health services, post partum, Primary Health Care, Puskesmas, Maternal Mortality Rate

Abstract

Background : The city of Semarang is included in the top 5 highest maternal mortality rates in Central Java. The Semarang City Government through the Semarang City Health Service seeks to reduce maternal mortality through the establishment of community health centers capable of Basic Emergency Obstetric and Newborn Care (BEMONC) with the support of the establishment of the Ambulan Hebat program. Bangetayu Community Health Center is a community health centers capable of BEMONC that continues to contribute to maternal mortality in the last 3 years with the highest number of high risk pregnant women and the largest number of complications pregnant women. This study aims to analyze the opportunities for synchronizing the Ambulan Hebat program in supporting the implementation of community health centers capable of BEMONC at the Bangetayu Community Health Center.Method : Qualitative research with a descriptive analytic approach through in-depth interviews. The variables studied were elements of partnership (shared interests, openness & trust, clear & measurable goals, willingness to sacrifice, and mutual benefit) by using analysis of supporting factors and inhibiting factors (Force Field Analysis). Result : The results of this study indicate that the Ambulan Hebat Si Cepat program in supporting the implementation of obstetric and neonatal services in the Bangetayu Community Health Center based on the element of partnership has a great opportunity to synchronize. There are 4 elements of partnership that are supporting factors are the aspects of openness & trust, clear & measurable goals, willingness to sacrifice, and mutual benefit.Recomendation : The results of this study suggest that the Semarang City Health Office make policy regulations to regulate program coordination, conduct routine internal coordination, and create operational standards for special coordination procedures.

Abstract

Accreditation assistance is an activity that aims to facilitate puskesmas in order to improve quality and service performance in accordance with standard and accreditation instrument of puskesmas. The score result at 14 puskesmas targeted for assistance in 2016 and 2017, most are accredited basic and showed the lowest score in the administrative and management working group. The objective of this study is to analyze the implementation of puskesmas accreditation in the administration and management working group in Pemalang. This study uses qualitative method with descriptive approach. The data was collected through in-depth interviews to two key informants and three triangulation informants. The results showed that in general the implementation of puskesmas accreditation assistance in Pemalang has been running, but if it is seen the quality of the implementation was still less than the maximum. There were still members of assistant team who have not been trained and have a competence assistant certificate; the activity planning did not detail the whole of the assistance activities but only focused on budget planning and scheduling activities; the implementation of accreditation assistance activities was limited for pre accredited certification assistance. The advice given is to facilitate education and training of untrained team members, making planning activities based on evidence data freom the result of self assessment so that assisstenace more directed; create a clear monitoring and evaluation schedule related to the implementation of accreditation assistance.

Abstract

Minimum Service Standards have to be provided by each region to their citizen regarding the minimum type and quality of basic services. Minimum health service standard have 12 type of basic services. One of them are the health service of hypertension patient which was only 24,75% in 2017 from target 100% in Semarang. The research objective was to analyze the application of minimum service standards. This research used a qualitative method with a descriptive analytic approach. Data collected were primary data from in-depth interviews and secondary data that were relevant to the research needs. The study was conducted at Gunungpati Health Center, Ngesrep Health Center, Pudak Payung Health Center, Sekaran health Center, and Semarang City Health Office with research subjects as Head of Health Center, Head of Administration, Responsible PTM Program Holder, and patients. The results showed the Minister of Health?s Regulation Number 4 of 2019 concerning Minimum Service Standards in the Health sector was too ideal and was it difficult to reached. Puskesmas as the executor of the Minister of Helath Regulation Number 4 of 2019 must have innovations in order to achieve the 100% target.

Abstract

There is a gap between the implementation of the first 1000 days of life Program on the Implementation Guidelines in Semarang City, among others health workers are not well informed about the program, there is no specific structure in the implementation of first 1000 days of life program in Semarang City in accordance with the first 1000 days of life program Implementation Guidelines, and there has not been an integrated step from the Health Department related to the first 1000 days of life program, therefore it is necessary to analyze stakeholder mapping related to its role and function in supporting the 1000HPK program.This research is an observatioal research with qualitative method. Respondents in this study were representatives of stakeholders involved in the 1000HPK Program in Semarang City. In determining the respondents, the researcher uses purposive sampling to assess stakeholder perceptions on the level of influence of power, level of interest, and attitudes of each stakeholder linked to the role of each stakeholder and process indicator of the first 1000 days of life program.The modification group of the stakeholder classification model is as follows: Decision makers include: Regional People's Representative Assembly of Semarang City and BAPPEDA Semarang City, Provider: Health Office of Semarang City, IBI (Indonesian Midwives Association) Semarang City, IDI (Indonesian Doctors Association) Semarang City, Ministry of Religious Affairs Semarang, Education Office of Semarang City, and Academics. Client: PHC, Pregnant Women, Breastfeeding Mothers, Cadre (Dasawisma). Representative: FMM (Forum Masyarakat Madani), Business World, Print Media, Television Media.The results of identification of stakeholders of decision makers and client groups, all states support for the first 1000 days of life Program Process Indicators, but for the provider stakeholders there are groups in the "Time Bomb" position indicating that the stakeholders have not fully supported and directly involved in the first 1000 days of life Program.
