Abstract

Background: Indonesia ranked the seventh position of the most diabetes patients among other countries. Several risk factors affected the viability of diabetes mellitus patients, such as sex, age, type of DM, obesity, diet program, and family history of DM. Purpose: This research aims to analyze the variables that affect the viability of diabetes mellitus patients with complications. Methods: This research used descriptive analytic research with cross-sectional research design. The population used in this research was diabetes mellitus patients with complications (hyperglycemia, ketoacidosis, and gangrene) who were hospitalized at X Hospital in Madiun, which recorded in January-December 2018. This research used a total sample with data obtainment techniques used medical records data sources of diabetes mellitus patients with complications (hyperglycemia, ketoacidosis, and gangrene) which recorded in January-December 2018 and observed until January 2019. The analysis technique used was survival analysis using Kaplan Meier, Breslow, and Cox regression. Results: The characteristics of respondents were the majority were 60 years old and over (53.70%) and were female respondents (63%). Based on the research results of several variables, only diet program variable which was significant among all independent variables (p = 0.01; HR = 3.74; 95% CI = 1.65 <OR <8.48), which suspected to affect the viability of diabetes mellitus patients with complications (hyperglycemia, ketoacidosis, and gangrene). Conclusion: The most influential risk factor is diet program.

Abstract

Acute Respiratory Infections (ARI) is a disease that occurs in many developing countries and also on the list of 10 most diseases in hospitals. This study aims to analyze the information system of ARI Reporting Program in Surabaya. Reporting on ARI cases is a surveillance of the disease. If many cases reported, it is better than only a few cases reported. This research is a descriptive study with a qualitative approach. Based on the results of the interview with the informant, it was stated that the ARI reporting program had helped in recording ARI cases that occurred in Surabaya. File formats that are still manual and not integrated with the internet (online) make sending reports longer. Even if you implement an integrated online, data entry only needs to be done once so that the same data will be stored electronically and can be sent and processed. In addition, in the excel file ARI reporting program, there are several output menus that can be used as a result of the description of ARI cases. However, the output was underutilized so that less case monitoring was implemented to reduce the number of ARI / Pneumonia cases that occurred in Surabaya. The conclusion is the ARI reporting program is very helpful in the process of data collection of ARI cases, but it would be better if they use the type of online computerization management and better utilize the graphical output of the ARI reporting program.

Abstract

Prognosis of cancer depends on variables, other factors, the stage of cancer, the biological warfare and general conditions when the cancer is diagnosed. Social status, economic status, and demographic issues choose in determining the stage of cancer when the patient first comes to the hospital. The purpose of this study to examine the role, nutritional status, and family history with breast cancer patients in Dr. Soetomo hospital. The study conducted in this study was a non-reactive or non-intrusive method. The sample in the analysis using simple random sampling with sample size of 95 patients. Does not contain the effect of variables associated with grade of breast cancer with p-value 0.795. While for variable of nutritional status and family history with cancer to breast cancer level with p-value 0.033 and 0.005. The p-value in the fitting information table was 0.003 model which contains not only the intercept that was not displayed. The value of Nagelkerke 0.157 or 15.7% means that variable cost, nutritional status and family history with cancer can be used only by 15.7%. From the existing variables was 2 significant variables, namely nutritional status with p-value 0.033 and family history with cancer with p-value 0.005. Abstract

Forecasting is a systematic attempt to predict future events usingpast data, based on scientific and qualitative methods. For the maternal health program, forecasting is important as its process consists of planning, targetting and achievement. Based on data from the Ministry of Health, the quality of antenatal care in Indonesia was still low (87.48 percent) compared to that of the national target (95 percent). This study aims to apply the methods of artificial neural network in predicting the antenatalcare (K4). This applied research used a descriptive method with secondary data in the form of monthly antenatal care visits (K4) from the year of 2012 to2015 obtained from the Provincial Health Office of East Java, with a case study in Bondowoso. The forecasting result in 2016 based onthe 12-4-1 network architecture was 9533.5698, with the value of Mean Square Error (MSE) of 3091.84404. The average percentage of errord based on a comparison with the actual data is 0.1854 or reaching the accuracy of 99.81 percent. The conclusion of this study is that a neural network has a low error value and a high accuracy.Therefore, forecasting results can be used as an input in the planning program.

Abstract

Religion and Spirituality can provide a unique perspective on life outcomes. The study to deepen subjective well being by exploring its relationships with religious and spirituality (secular values) and characteristics individu by the graph. Data were obtained from survey. The sample was drawn from the population aged 70 or more years in Surabaya by simple random sampling. The instrument using the Philadelphia geriatric center morale scale (PGCMS) and the self spirituality and religious. Most respondents reported being 'low of religiousity and spirituality? in their lives. The mean PGCMS score was 7.89. The median PGCMS score was 8, 57.4% (139) score PGCSM above the median and 42.6% (103) score PGCSM under the median. Among elderly who were included the high well being category (PGCSM?10) was 27.2% and most of elderly 72.7% didn?t have good well being. The elderly people who describe themselves as religious are showed graph constant. The graph of self religious is equal according to characteristic individu (age, men and women, marital status). The elderly who describe themselves as spiritual are likely to report greater or lower perceived well being. Exploring using the graph showed different according to characteristic individu. Elderly who demonstrated subjective well being were more likely to male and not married. Elderly?s perception of spirituality depends on characteristic individuals and experience in which individuals live.

Abstract

The maternal mortality rate (MMR) in developing countries is still a major health problem, including in Indonesia. Antenatal Care (ANC), delivery with skilled birth attendance (SBA) at the time of delivery and delivery in institutional are universally considered important for reducing maternal mortality. Husbands can play a crucial role in pregnancy and childbirth. Therefore, the aim of this study was analyze the indicators of husband's role in pregnancy and maternity care which were suspected as one of the determinants of   ANC and SBA use in Polewali Mandar Regency. The population of this prospective cohort study was all married and pregnant woman, before using contraceptives and contraceptive failure or not using contraception and pregnancy is planned, unplanned pregnancy or mistimed pregnancy in Polewali Mandar 2015 (Size of sample = 100). Samples were randomly selected from participants of screening in 12 sub-districts in Polewali Mandar. Data about husband's role in pregnancy and maternity care was obtained through interviews using a structured questionnaire data processing by using SMART-PLS.  The result of data analysis suggested that the coefficient value that has been standardized from each indicator were as follows: accessibility = 0.944 and engagement = 0.954, dan responsibility = 0.968. Indicators of organizational support in implementing Maternal and Child Health Information System at Polewali Mandar Regency, respectively from the most important are: responsibility, engagement, and accessibility.

Abstract

Survival analysis as a collection of statistical procedures for analyzing the data that its outcome variable was the time to occurrence of an event. Kaplan-Meier method is a type of survival analysis technique, this method is often called the Product Limit Method. Chronic Kidney Disease (CKD) became one of the public health problem throughout the world, including Indonesia. The number of hemodialysis patients has increased every year and have an impact on increasing the number of death in General Hospital Ibnu Sina Gresik. This study was determine the survival of hemodialysis patients using Kaplan-Meier analysis techniques. Non-reactive research with a retrospective cohort using the calculations right censoring. 155 population were taken randomly and sample size of 111. Data were collected using a checklist. The estimated survival time of female, adult age, further education, patients work, patients without insurance, patients with normal nutritional status, patients with a history of disease, patient with hypertention and patient with diabetic had a better survival time. The insurance status, nutritional status, hypertension, and diabetes mellitus were significant difference to the survival time (p-value &lt;0.05). It was necessary special treatment for CKD patients through giving information, education to families and patients to maintain healthy lifestyle.

Abstract

The prevalence and death due to low birth weight (LBW) is still high in Kupang. There are multifactorial causes of LBW which can be categorized into: physical and social environment, mother, fetus, placenta and umbilical cord, and health service. From maternal factors, there are sub-factors of maternal characteristic such as knowledge of pregnancy, maternal health status, and the behavior of pregnancy care. The behavior of pregnancy care can be in the form of a traditional pregnancy care, which may lead to LBW. Most researches on the risk factor of LBW are related to clinical medical factors. This research aimed to determine the socio-demographic and cultural factors which are related to the occurrence of LBW in Kupang. This research used analytic observational method with case-control design, on 50 mothers giving birth to LBW and 50 mothers who delivered normal babies, through interview and tracing of maternal and infant medical records. To eliminate the clinical factors of medical causes of LBW (anemia, hypertension, pre-eclampsia, and infection), inclusion criterion was applied, as follows: mothers who give birth to normal childbirth, single infants, and have received integrated Antenatal Care (ANC) service. Exclusion criteria were: mothers give birth to twisted umbilical cord, placental weight <500 grams, infant with congenital defect, and incomplete medical records. This research took place at Prof. DR. W.Z. Johannes Hospital, Dedari Hospital and 2 Public Health Center in Kupang. Data analysis used logistic regression. The final results of logistic regression model of this study founded 5 variables that affect the occurrence of LBW in Kupang, beside the medical clinical factors, such as: maternal age, maternal education, BMI, increase the body weight during pregnancy, and heavy work during pregnancy. Abstract

Bayi berat lahir rendah (BBLR) adalah berat lahir kurang 2500 gram tanpa memandang umur kehamilan. Penyebab BBLR multifaktoral, faktor ibu yang berpengaruh secara dominan mengenai pemenuhan nutrisi/ asupan gizi. Faktor plasenta yang berguna untuk menyalurkan makanan dan pembentukan hormon.Tujuan penelitian menganalisis hubungan antara faktor-faktor yang berhubungan antara status gizi dengan berat plasenta pada ibu yang melahirkan BBLR. Status gizi ibu hamil meliputi penambahan berat badan, ukuran LiLA, kadar Hb awal dan kadar Hb inpartu. Jenis penelitian ini adalah observasional deskriftif dengan pendekatan cross sectional study. Hasil penelitian nilai π = 0,097, nilai ρ = 0,360 ukuran LiLA, nilai ρ = 0,394 kadar Hb awal dan nilai ρ = 0,100 pada kadar Hb inpartu dengan berat plasenta pada ibu yang melahirkan BBLR. Dari hasil kekuatan hubungan, yang paling kuat adalah ukuran LiLA dan kadar Hb awal, artinya pentingnya status gizi di awal kehamilan, karena pada trimester pertama kehamilan adalah proses pembentukan organ vital janin dan plasenta. Petugas kesehatan melakukan pengukuran berat plasenta, nasehat makan makanan bergizi. Penelitian lebih lanjut tentang berat plasenta antara persalinan aterm dan preterm pada BBLR. Kata kunci : Status Gizi, Berat Plasenta, BBLR, Pertambahan Berat Badan, ukuran LiLA, Kadar Hb Awal dan Kadar hb Inpartu

Abstract

ABSTRACTDefault relating to the regularity of treatment in the patients taking the drugs. If the leprosy patients taking medication irregularly, then the leprosy bacteria can become resistant or immune to Multy Drug Therapy, so the symptoms persist and even worsen. The purpose of this study was to determine the indicators and develop a predictive index formula default incidence of leprosy treatment. This type of research was a case control design. The sample size in this study was 62 respondents. The sampling technique in this study used simple random sampling technique. Data collection techniques done with interviews with the questionnaire. The results showed that a candidate indicator variables (p &lt;0.25) is income (p value = 0.013), access to health care (p value = 0.022), the role of health workers (p value = 0.032), knowledge (p value = 0.001), and family support (p value = 0.023), perception of the disease (p value = 0.098), leprosy reactions (p value = 0.198). Variables as indicators of default treatment of leprosy patients are the role of health workers, family support, knowledge, and income. The predictive index of default treatment at multibacillary leprosy patients are (-1666+ 1451 * Revenues (low) + 1989 * Role of Health Personnel (less instrumental) + 1.338 * Knowledge (less) + 1543 * Support Family (unfavorable). This index has a 79% accuracy rate. This index is expected to be used as a measuring tool in helping health workers to screen patients with leprosy who are undergoing treatment, so early can know where the potential patients defaulting treatment.Keywords: indicators, predictive index, default of treatment, leprosy, multibacillary
