Abstract

Endometriosis is one of the most common gynecological problem. Cells resulted in chronic inflammation and progressive, proliferative, invasive and even infiltrating an area that resembles the character of the malignancy. Ki-67 is an antigen on the cell nucleus that is found only in actively dividing cells. Expression of Ki-67 are associated with an aggressive tumor and metastasis. This study aims to determine the level of Ki-67 expression correlation with stage and size of the endometriosis cyst. Methods research is observational analytic cross cut method on 56 paraffin blocks of patients who have been diagnosed with endometriosis and had performed a laparotomy or laparoscopic surgery in Dr Hasan Sadikin Hospital. The results showed a significant relationship between the level of expression of Ki-67 with endometriosis cyst size (p <0.001) with a fairly strong relationship (0.55) according to statistics based on criteria Guilford. Moreover the results also showed a significant relationship between the level of expression of Ki-67 with endometriosis stage (p <0.001) with a fairly close relationship (0.564) according to statistics based on criteria Guilford. It can be concluded that the expression of Ki-67 associated with cyst size and stage of endometriosis. Keywords: Ki-67, endometriosis stage, endometriosis cyst

Abstract

Background: Mindi leaves (Melia azeradach L.) were used by Indonesians as a traditional medicine for pregnant women because it was considered to be safe. Mindi leaves contain several active compounds and one of them is suspected as a teratogen and can disrupt fetus growth in gestation. This research aims to know about the teratogenic effect of ethanol extract of Mindi leaves by using mice.Methods: This was a laboratory experimental study using 27 pregnant female mice (Mus musculus) of Swiss Webster strain which were randomly assigned to 3 groups (n=9) controlled (Carboxymethyl cellulose 1% for day 1–18 of pregnancy), group I (mindi leaves ethanol extract 3.22 mg+Carboxymethyl cellulose 1% day 1–5 of pregnancy), and group II (mindi leaves ethanol extract 3.22 mg+Carboxymethyl cellulose 1% day 6–18 of pregnancy). Observation was done to see total amount of fetus, live normal fetus count, length and weight of fetus, abnormal fetus count consisting of dead fetus count with normal and abnormal external morphology, and resorbed fetus count. This research was done from October to November 2012 in Pharmacological laboratory of Faculty of Medicine Universitas Padjajdjaran. Data analysis utilized unpaired t-test.Results: The result showed a significant difference (p<0.05), seen from live normal fetus count and abnormal fetus count consisting of dead fetus count with normal and abnormal external morphology, and resorpted fetus count. Conclusions: Administration of Mindi leaves extract during pregnancy of mices can cause teratogenic effect.[AMJ.2015;2(1):221–25]

Abstract

PendahuluanAngka Kematian Ibu (AKI) merupakan salah satu indikator untuk melihat derajat kesehatan perempuan. Selain itu AKI merupakan salah satu target utama yang telah ditentukan oleh WHO sebagai indikator kesehatan suatu negara.1  Dari hasil survey yang dilakukan, AKI di Indonesia telah menurun dari waktu ke waktu, namun masih relatif tinggi dibandingkan negara Asia lainnya. Jumlah angka kematian ibu di Indonesia (angka nasional) tahun 1991 sebanyak 390 sedangkan pada tahun 2015 menurun mencapai 305/100.000 jumlah kelahiran hidup.2 Di sisi lain, angka kematian ibu Provinsi Jawa Barat tahun 2015 adalah sebanyak 823/100.000.3Rendahnya kesadaran masyarakat tentang kesehatan ibu hamil menjadi faktor penentu angka kematian, meskipun masih banyak faktor yang harus diperhatikan untuk menangani masalah ini. Persoalan kematian yang terjadi antara lain pendarahan, preeklamsi-eklamsi dengan komplikasi, aborsi, dan infeksi. Namun, ternyata masih ada faktor lain yang cukup penting, yaitu pemberdayaan perempuan yang belum baik, latar belakang pendidikan, sosioekonomi keluarga, lingkungan masyarakat dan kebijakan publik. Kaum lelaki pun dituntut harus berupaya lebih aktif dalam segala permasalahan bidang reproduksi. Oleh karena itu, pandangan yang menganggap kehamilan adalah peristiwa alamiah perlu diubah secara sosiokultural agar perempuan dapat lebih mendapat perhatian dari masyarakat. Sangat diperlukan upaya peningkatan pelayanan perawatan ibu oleh pemerintah, swasta, maupun masyarakat lainnya terutama suami.Kematian ibu disebabkan oleh hipertensi dalam kehamilan (HDK) secara global menempati nomor dua setelah kasus perdarahan, demikian pula di Indonesia.4 Pada tahun 2016 dalam rangka menunjang kegiatan penurunan angka kematian ibu, Perkumpulan Obstetri dan Ginekologi Indonesia (POGI) bersama Kantor Kementerian Kesehatan mengeluarkan Panduan Nasional Praktik Kedokteran (PNPK) tentang preeklamsi yang menjadi dasar untuk pembuatan standar pelayanan preeklamsi di seluruh Indonesia dan diharapkan mampu membantu mempercepat penurunkan AKI.

Objectives: To analyze the association between Ki-67 expression with the cyst size and stage of endometriosis, and the correlation strength between them. Methods: A cross-sectional analytic observational study involving 56 paraffin blocks from subjects diagnosed with endometriosis, who had undergone laparotomy or laparoscopic surgery. The study is conducted in Dr. Hasan Sadikin Hospital in September-November 2012. Results: Shows a significant association between Ki-67 expression and the size of endometriotic cyst (p &lt; 0.0001), also with a strong correlation (r = 0.55) according to Guilford criteria. There is also a significant association between Ki-67 expression and the endometriosis stage (p &lt; 0.0001), with a strong correlation (r = 0.564) according to Guilford criteria. Abstract

AbstrakTujuan: Penelitian ini dilakukan upaya untuk menilai karakteristik dan outcome kehamilan dengan kelainan jantung baik pada ibu dan bayi di RSUP dr. Hasan Sadikin Bandung dari tahun 2015 sampai 2017.Metode: Penelitian ini merupakan penelitian observasional analitik (cross-sectional). Subjek penelitian adalah semua ibu hamil dengan kelainan jantung yang menjalani persalinan di RSUP dr. Hasan Sadikin Bandung dari tahun 2015 ? 2017 dengan menggunakan data sekunder. Hasil: Selama periode penelitian sebanyak 76 sampel penelitian yang diperoleh. Pada penelitian ini usia rata-rata ibu adalah 30 tahun, paling banyak pada paritas 1 dan 3 yaitu sebesar 31,6%. Usia kehamilan saat terjadi persalinan  > 37 minggu sebanyak 53,9%. Lama perawatan pasien rata-rata 7 sampai 8 hari dengan ruang rawat yang paling banyak adalah ICU sebanyak 32,9%, dan ruang rawat biasa sebesar 39.5%. Jenis kelainan jantung yang paling sering adalah kardiomiopati peripartum dan hipertensi, yaitu sebesar 42,1%. Jenis persalinan yang banyak dilakukan adalah seksio sesarea yaitu sebesar 64,5%. Penelitian ini memperoleh bahwa ibu dengan kelainan jantung yang hidup sebesar 88,2% dan meninggal sebesar 11,8% setelah menjalani persalinan.Kesimpulan: Ada perbedaan antara derajat kelainan jantung berdasarkan NYHA (New York Heart Association) dengan outcome ibu dan bayi pada kehamilan dengan kelainan jantung.Kata kunci: Jenis dan Derajat Kelainan Jantung, Jenis Persalinan, Outcome Ibu dan BayiAbstractObjective: through this research conducted efforts to assess the characteristics and outcome of pregnancy with cardiac disease both in mother and infant in dr dr. Hasan Sadikin Bandung from year 2015 to 2017.Method: an observational analytic (cross-sectional) research with subjects were all pregnant women with cardiac abnormalities who underwent delivery at dr. Hasan Sadikin Bandung from 2015 to 2017 using secondary data.Results:During the research period as many as 76 samples obtained. In this study the average age of the mother is 30 years, most at parity 1 and 3 that is equal to 31.6%, age of pregnancy during labor> 37 weeks by 53.9%. The average length of patient care was 7 to 8 days with the most hospital room was ICU of 32.9% and the regular room was 39.5%. The most common types of heart disorders are peripartum cardiomyopathy and hypertensive heart disease, which is 42.1%. Type of delivery mostly by cesarean section that is equal to 64.5%. The study found that mothers with heart abnormalities were 88.2% alive and died of 11.8% after going through labor.Conclusion: There is a difference between the degree of cardiac abnormalities based on NYHA (New York Heart Association)  classification with maternal and infant outcomes in pregnancy with cardiac abnormalities.Key words: Type and Degree of Heart Abnormality, Type of Birth, Outcome of Mother and Infant

Abstract

Latar Belakang: Preeklamsi berat merupakan salah satu penyebab tersering mortalitas dan morbiditas pada ibu dan bayi. Morbiditas dan mortalitas pada ibu akan semakin meningkat karena terjadinya komplikasi. Komplikasi preeklamsi berat yang terjadi dapat berupa komplikasi kardiovaskular, hati, otak dan ginjal. Tujuan penelitian ini untuk mencari perbedaan kadar asam urat, laktat dehidrogenase atau LDH dan aspartat aminotransferase atau AST pada serum penderita preeklamsi berat disertai komplikasi dan tanpa komplikasi dan mengukur kuatnya hubungan peningkatan kadar asam urat, LDH dan AST dengan peningkatan risiko terjadinya komplikasi pada pasien preeklamsi berat.Metode: Rancangan penelitian ini adalah penelitian comparative cross sectional dengan metode consecutive sampling yang membandingkan hasil laboratarium asam urat, LDH, AST dari subjek penelitian yang memenuhi kriteria inklusi. Subjek penelitian adalah penderita preeklamsi berat disertai komplikasi dan preeklamsi berat tanpa komplikasi (n=68). Penelitian dilakukan di Rumah Sakit Dr. Hasan Sadikin Bandung dan rumah sakit jejaring pada bulan November-Desember 2016.Hasil : Hasil penelitian didapatkan perbedaan kadar asam urat, LDH dan AST pada kedua kelompok secara bermakna dengan nilai p ? 0,05. Peningkatan kadar asam urat, LDH dan AST berhubungan dengan peningkatan risiko terjadinya komplikasi pada preeklamsi berat dengan nilai cut off kadar asam urat > 6,5 mg/dL sebesar 33 kali, LDH > 573 U/L sebesar 8,95 kali dan AST > 30 U/L sebesar 5,19 kali. Jika terjadi peningkatan seluruh kadar asam urat, LDH dan AST diatas nilai cut off maka risiko terjadinya komplikasi pada preeklamsi berat sebesar 98,1%.Kesimpulan: Penelitian ini menyimpulkan kadar asam urat, LDH, AST pada preeklamsi berat disertai komplikasi lebih tinggi dibandingkan preeklamsi berat tanpa komplikasi dan peningkatan kadar asam urat, LDH, AST berhubungan dengan peningkatan risiko terjadinya komplikasi pada preeklamsi berat.
