Abstract

Breast cancer is the cancer with the most prevalence of 43.3% and the mortality rate that reaches 12.9%. The prognosis of breast cancer can be affected by diabetes mellitus (DM), so that efforts to control DM through antidiabetic therapy are very necessary, but antidiabetic therapy is also reported to be associated with the prognosis of breast cancer. The purpose of this study was to determine the relationship of diabetes and antidiabetic use to recurrence in breast cancer patients. This study used a retrospective cohort design, involving 176 female non-metastatic breast cancer patients who received chemotherapy, consisting of 88 patients with DM and 88 non-DM patients. The exposures in the study were diabetes mellitus and the types of antidiabetic drugs (metformin and non metformin based therapy), while the study output was the recurrence of breast cancer. The research data was obtained from the patient?s medical records at RSUP Dr. Sardjito Yogyakarta. Research analysis used Chi-square, Kaplan Meier method and Cox-regression to estimate Hazard Ratio with 95% confidence interval. The results showed DM in breast cancer patients was associated with increased the risk of recurrence (HR 2,458; 95% CI 1,571-3,846, log rank test P=0,000), while the use of antidiabetic types (metformin and nonmetformin) to control DM in breast cancer patients was not associated with the risk of recurrence (HR 1.391; 95% CI 0.816 - 2.370, log rank test P=0.210). Further research is warranted by monitoring blood glucose levels regularly.

Abstract

Penelitian bertujuan untuk menghasilkan lembar kerja siswa tematik integratif berbasis Problem Based Learning yang layak untuk meningkatkan karakter tanggung jawab siswa kelas IV SD, dan menguji keefektifannya. Desain penelitian yang digunakan adalah desain R&D Borg dan Gall. Penelitian dilakukan di SDN Semanu III, SDN Karangrejek II, dan SDN Gombang II, Kabupaten Gunungkidul. Instrumen pengumpulan data menggunakan pedoman wawancara, lembar observasi, lembar validasi ahli, angket respon guru, skala respon siswa, dan skala karakter tanggung jawab. Keefektifan produk diuji melalui quasi experiment. Data dianalisis secara despkriptif kualitatif dan kuantitatif melalui uji beda dengan taraf signifikansi 0,05. Hasil penelitian ini berupa lembar kerja siswa yang layak digunakan berdasarkan penilaian ahli media dan ahli materi dengan kategori ?sangat baik?. Produk yang dikembangkan efektif meningkatkan karakter tanggung jawab siswa. Hal ini dibuktikan uji beda postskala kelas eksperimen 1 dengan kelas kontrol perbedaan reratanya signifikan pada nilai t hitung 2,119 dan two-tails significance pada 0,040. Pada uji beda post skala kelas eksperimen 2 dengan kelas kontrol, perbedaan reratanya signifikan pada nilai t hitung 2,164 dan two-tails significance pada 0,036 dengan taraf signifikansi 0,05.Kata kunci: lembar kerja siswa tematik integratif, problem based learning, karakter tanggung jawab STUDENT WORK SHEET BASED ON THE PROBLEM BASED LEARNING TO IMPROVE RESPONSIBILITY STUDENTS OF ELEMENTARY SCHOOLAbstractThis study aimed at producing a suitable thematic-integrative student work sheet based on the Problem Based Learning to improve the critical thinking ability and responsibility character of grade IV students of SD, and testing the effectiveness of student work sheet. This study used the R&D design of Borg and Gall. The study was conducted at SDN Semanu III, SDN Karangrejek II, and SDN Gombang II, Gunungkidul Regency. The instrument data were collected using an interview guide, observation sheet, validation expert sheet, questionnaires of teachers responses, scale of students responses, and scale of responsibility character. The product effectiveness was tested by using a quasi experiment. The data were analyzed descriptive-qualitatively and quantitatively through  t  test  with the a significance level of 0,05. This research produced a student work sheet suitable for use based on assessment by media and subject matter experts with ?very good? category. The product could improve the responsibility character of students. The mean score difference was significant at 2.119 t test value  with the two-tails significance standing at 0.040 for t test of the experimental group 1 with the control group.  On the t test of the experimental group 2 with the control group, the mean score difference was significant at 2.164 t test value  with the two-tails significance standing at 0.036 with the a significance level of 0.05.Keywords: Thematic-integrative student work sheet, Problem Based Learning, responsibility character

Abstract

Drugs? administration among hospitalized patients in ICU commonly is given intravenously. Mixing the intravenous drugs may result in incompatibility problem that might affect the drugs? stability and bioavailability. The aim of the study was to investigate the potential incompatibility problem of  intravenous mixing drugs? administration among ICU patients at PKU Muhammadiyah Yogyakarta Hospital. This study was a cross-sectional study in which design and data was obtained from ICU patients? medical record retrospectively with purposive sampling in order to observe the pattern of intravenous drug?s combination. The potential incompatibility problem was analyzed using Handbook on Injectable Drugs. There were 79 out of 119 medical records which fulfilled sample inclusion criteria taken in this study. Parenteral dosage form was commonly used rather than non parenteral (62.06%) among ICUs? patients. The potential incompatibility pattern consisted of incompatibility of intravenous drugs, electrolyte solutions/parenteral nutritions in mixture form, and the electrolyte solutions/parenteral nutritions, which are administrated simultaneously. Potential incompatibility of intravenous dosage was found in 50 events out of 79 patients (0.63 events per patients), which consisted of 8 events (8.51%) in using of drugs administrated simultaneously, 10 events (19.23%) in using of electrolyte solutions/parenteral nutritions in mixture form, and 32 events (11.72%) in using of electrolyte solutions/parenteral nutritions administrated simultaneously. Common potential incompatibilities types were precipitation of drugs and drug?s adsorption to packaging materials.

Abstract

Adverse drug reactions (ADRs) merupakan masalah yang sering terjadi pada pasien lanjut usia. Penelitian bertujuan mengetahui angka kejadian adverse drug reactions pada pasien rawat inap geriatri.Penelitian dilakukan dengan mengambil data secara prospektif pada 100 pasien rawat inap geriatric dengan kriteria usia pasien 60 tahun atau lebih dan menjalani rawat inap di bagian Penyakit Dalam (IRNA1) RSUP Dr. Sardjito Yogyakarta pada dua periode waktu (Januari–Februari 2006 dan Agustus–Oktober 2006). Pengambilan data dilakukan melalui catatan medic kemudian identifikasi efek samping obat dilakukan melalui diskusi farmasis dengan klinisi konsultan geriatri. Evaluasi data dilakukan secara descriptive Hasil penelitian menunjukkan bahwa ADRs diduga kuat dialami oleh 25 pasien dengan total 30 kejadian ADRs. Sebagian besar ADRs yang ditemukan berhubungan dengan gangguan pada gastrointestinal (mual, muntah, dispepsia, konstipasi, diare dan gastritis) 56,7.% dan kardiovaskular (hipertensi dan hipotensi) 16,7.%. Farmasis dapat berperan dalam identifikasi ADRs melalui kolaborasi dengan tenaga kesehatan lain di rumah sakit sehingga dapat dilakukan upaya pencegahan terhadap ADRs pada pasien geriatriKata kunci: Adverse drug reactions (ADRs), Geriatri, Rawat inap.

Abstract

A research has been carried out to identify whether prescriptions received in pharmacies at Yogyakarta district fulfilled the legality as the regulation concern. This included an investigation on prescription hand-writing, part of a prescription which has a potential to bring about a medication error due to misinterpretation.The study was conducted by collecting prescription samples, randomly picked-up (=5% and d=3%), from each of 12 pharmacies. Furthermore, other supporting data, such as questioner and interview, were also collected from 29 pharmacies involving 83 respondents (24 pharmacists and 59 technician assistants). Data were then analyzed descriptively.The result showed that prescriptions fulfilled the regulation was approximately 39.8%. The absences of sign or license number of prescriber, the date of prescription were only examples of the cause. Hand-writing of the prescriber was sometime so bad that causing difficulty to interpret especially name of medicine, dose, usage or administration, uncommon abbreviation, hence increasing the risks of medication error to happen.Key word: prescription, legality aspect of prescription, medication error

Abstract

The increasing number of drugs available and the increasing use of multidrug regimens which enhance the possibility for drug interactions. The aim of the study was to determine the frequency of drug-drug interaction and drug-food interaction both in outpatients and hospitalized patients. The research was also to determine the pattern of drug interaction mechanism and drugs which often show interaction.The Research type was descriptive. The data were conducted by retrospective, simple random sampling, taken from 90 samples of geriatrics patient hospitalized medical record in Interne medicine during June- September 2003, 120 samples of outpatient prescription took care from Haspita Farma Pharmacies during July-December 2003. Evaluation of the data was carried out descriptively.The result of the study showed that drug interaction occurred in 59% cases of hospitalized patient and 69% cases of outpatient. It was identified the existence of 125 drug interaction in hospitalized patient consists of 48 drug-drug interaction and 77 drug-food interaction. The pattern of drug interaction mechanism was pharmacokinetics pattern of drug interaction mechanism 36%, pharmacodinamic 16 % and unknown 48 %. The drug which often have interaction were furosemid, captopril, aspirin, and ceftriakson. In outpatient, It was identified 128 drug interactions consist of 47 drug-drug interactions case and 81 drug-food interaction. The mechanism pattern was pharmacokinetic 72%, pharmacodinamic 19% and unknown 11 %.The drug which often had interaction was phenitoin, phenobarbital, isoniasid, and rifampicin.Key words : drug interaction, Dr. Sardjito Hospital, Geriatric

Abstract

Hypertension  in  patients  with  type  2  diabetes  mellitus  is  a  prevalent condition that leads to substantial morbidity and mortality. The aim of the study was  to  determine  the  prevalence  of  hypertension  in  type  2  diabetes  attending endocrinology  clinics  at  Hospital  Universiti  Sains  Malaysia  (HUSM),  to  describe antihypertensive  treatment  patterns,  and  to  evaluate  the  target  of  blood pressure  achieved  by  antihypertensive  agents.  The  study  was  a  prospective follow-up  descriptive  study.  Study  conducted  at  diabetic  clinic  outpatient  at HUSM,  Kelantan,  Malaysia  from  January  to  December  2007.  A  total  of  1077 diabetic patients involve in this study. The findings of our study showed that the majority patients type 2 diabetes mellitus 998 (92.7 %) had hypertension. For the  patients  who  were  on  antihypertensive  medications,  254  (23.6  %)  of  them were  on  monotherapy  and  311  (28.9  %),  179  (16.6  %), and  198  (18.4  %) respectively  received  two,  three  and  more  than  three  antihypertensive  agents. The majority of patients 872 (87.4 %) were on angiotensin converting enzyme inhibitors/angiotensin receptor blockers either alone or in combination with other antihypertensive  drugs.   A  number  of  601  (55.8  %)  patients  achieved  blood pressure  targets  ≤  130/80  mm  Hg.   Almost  all  patients  with  hypertension  anddiabetes will require a combination of antihypertensive medications. A number of patients was not achieved blood pressure goals ≤ 130/80 mm Hg. There is still need  to  educate  both  patients  and  health  care  providers  of  importance  of achieving target of blood pressure in order to reduce morbidity and mortality in diabetes mellitus patients.Key words: Hypertension, type 2 diabetes mellitus, hypertension treatment

Abstract

The study aims to understand the students’ learning motivation at Class V of the Indonesiancourse by applying a Cooperative Script strategy with comic media. It was aclass action research. The population included all the 17 students of the Class V at SDNegeri II Gedong Primary School. It consisted of two cycles. The data collection employedobservation and documentation. The technique of data analysis used a qualitativemethod with an interactive model, including data reduction, display data, and conclusion.The indicators were summarizing, listening, speaking, and expressing opinions.The class action research would be said to be successful if it met 80% of the indicatorsand 80% of the students achieved the KKM. The results of the study were summarizingat the early condition of 90.98%, cycle I of 74.51%, cycle II of 88.24%; listening at theearly condition of 50.98%, cycle I of 72.54%, cycle II of 90.20%; speaking at the earlycondition of 45.09%, cycle I of 64.71%, cycle II of 80.39%; and giving opinions at theearly condition of 43.20%, cycle I at 49.02%, cycle II of 80.39%. At the early condition,the students who achieved the KKM amounted to 9 people, at cycle I of 11 students, andat cycle II of 17 students (or 100%). At the cycle II, the students’ motivation had achievedthe indicators. It could be concluded that applying a cooperative script strategy withcomic media could increase the students’ Indonesian course.

Abstract

Drug Related Problems (DRPs) merupakan masalah yang berhubungan dengan obat yang banyak terjadi di pelayanan kesehatan. Tujuan penelitian ini untuk mengetahui profil kejadian DRPs, profil antibiotik yang mengalami DRPs, dan hubungan antara Drug Related Problems penggunaan antibiotik dengan luaran terapi. Penelitian ini merupakan penelitian observasional analitik dengan rancangan retrospective cohort study. Subyek penelitian yang memenuhi kriteria inklusi sebanyak 128 pasien, dimana terdapat 78 kejadian drug related problems pada 64 pasien dengan rincian sebagai berikut; indikasi tanpa obat 0 kejadian, obat tanpa indikasi yang sesuai 5 kejadian (6,41%), pemberian obat tidak tepat 1 kejadian (1,28%), dosis kurang 17 kejadian (21,79%), dosis lebih 7 kejadian (8,97%), adverse drug reaction 14 kejadian (17,95%), interaksi obat 33 kejadian (42,32%), dan kegagalan menerima obat 1 kejadian (1,28%). Terdapat dua antibiotik yang paling banyak mengalami DRPs, yaitu ceftriaxon dan cefotaxim. Pada uji Chi square didapatkan tidak ada hubungan antara jumlah kejadian DRPs terhadap luaran terapi dan lama waktu rawat inap (p>0,05).
