Abstract

ABSTRACTIntroduction. Old ages are associated with physiologic deteriorations of all organs and tissues, such as involution of thymus and decreased function of gastrointestinal tract which lead to malnutrition. Involution of thymus will caused reduction of T cell subset for instance T cell CD4+. Prevalence of nutrient deficiency in old age was high, according to a survey that 35% of subjects with age above 50 years old showed deficiency for one or more vitamins and trace-elements. Zinc as one of trace-elements has vital role for immune system.Aim. Aims of this study are to know the mean of zinc serum level, proportions of zinc deficiency in elderly and correlation between zinc and CD4+ count.Methods and Subjects. Design of this study is cross sectional,  with study subjects including healthy elderly women from Aisyiah elderly club at Kota Gede Yogyakarta. Elderly who were eligible with study criteria were measured height, weight, zinc serum level, and CD4+ count. Correlation test was performed to know correlation between ages, weight, body mass index (BMI), and zinc serum level with CD4+ count. Significant variables will be tested with multiple linier regressions. Level of significance is p < 0.05.Results. A total healthy elderly women subjects were 60 persons with median of age 63.5 years old. Median zinc serum level was 5.47 umol/L with range between 3.25-             7.95 umol/L. Zinc deficiency (zinc serum level < 7.65 umol/L) occured in 95% of subjects. Mean of CD4+ count was 767.22±232.95 count/mm3. There were correlations between age, weight, and zinc serum level with CD4+ count (r= -0.296; 0.345; 0.324; 0.792, respectively). Results from linear regression model showed zinc serum level was the only variable that affect to CD4+ count (adjusted R2 = 0.622).Conclusion. Ninety five percent of healthy elderly women at elderly club Aisyiah Kota Gede Yogyakarta suffer from zinc deficiencies, and there is strong correlation between zinc serum level and CD4+ count. Keyword: zinc serum, CD4+ count, elderly

Abstract

Increasing the secretion of reactive oxygen intermediates (ROI) by monocytes in the synovial fluid is anindicator to determine the severity of joint inflammation. Previous studies have shown that curcumin inhibitthe osteoarthritis progression with its ability to inhibite the activity of the nitric oxide synthase (NOS) enzymefrom macrophages. In this prospective randomized open end blinded evaluations = PROBE study, 80 patientswith knee osteoarthritis were eligable. The subject were devided in to two group: group who received 3 x 30mg of curcuminoid from Curcuma domestica Val. extract (curcuminoid group) and group who received 3 x 25mg of diclofenac sodium (diclofenac group) as comparison. The treatment was for 4 weeks time. The secretionof ROI by sinovial fluid monocytes was calculated by scoring the amount of formazan formation after neutralred staining in nitrobleu tetrazolium reduction assay. The result of this study showed that the secretion of ROIby synovial fluid monocytes was significantly decreased in both groups (p <0.001) respectively. There wasno significant difference in decreasing of ROI secretion of synovial fluid monocytes between both treatmentgroups (p = 0.92).

Abstract

Introduction: Mobility impairment is a problem in the older adults who have decreased in mobility as it may affect their daily activity. The development of a detection model to identify the problem of mobility impairment in older adults has become a solution that can increase the health care for older adults. This study aimed to develop a health detection instrument models using a mobile health nursing application to detect mobility impairment in older adults.Methods: This study used action research through a purposive sampling method involving three nurses and twenty-seven cadres to perform the detection process of mobility impairment focused on one hundred and seventy-five older adults in three public health centers in two provinces using an m-health application.Results: Based on direct observation and questionnaires addressed to the user of the m-health nursing application, 80% stated that the information contained in the mobile health nursing application was appropriate. In terms of speed, only 43.33% stated that the application worked fast, but overall, 66.67% of users stated that they were delighted with the application-based of the instrument model and that they were helped in detecting the mobility disorders that occurred in the older adults.Conclusion: These applications can be developed into a model that can help nurses, older adults and their family to detect other older adult problems in addition to mobility problems like cognitive function etc.

Abstract

Adverse drug reactions (ADRs) merupakan masalah yang sering terjadi pada pasien lanjut usia. Penelitian bertujuan mengetahui angka kejadian adverse drug reactions pada pasien rawat inap geriatri.Penelitian dilakukan dengan mengambil data secara prospektif pada 100 pasien rawat inap geriatric dengan kriteria usia pasien 60 tahun atau lebih dan menjalani rawat inap di bagian Penyakit Dalam (IRNA1) RSUP Dr. Sardjito Yogyakarta pada dua periode waktu (Januari–Februari 2006 dan Agustus–Oktober 2006). Pengambilan data dilakukan melalui catatan medic kemudian identifikasi efek samping obat dilakukan melalui diskusi farmasis dengan klinisi konsultan geriatri. Evaluasi data dilakukan secara descriptive Hasil penelitian menunjukkan bahwa ADRs diduga kuat dialami oleh 25 pasien dengan total 30 kejadian ADRs. Sebagian besar ADRs yang ditemukan berhubungan dengan gangguan pada gastrointestinal (mual, muntah, dispepsia, konstipasi, diare dan gastritis) 56,7.% dan kardiovaskular (hipertensi dan hipotensi) 16,7.%. Farmasis dapat berperan dalam identifikasi ADRs melalui kolaborasi dengan tenaga kesehatan lain di rumah sakit sehingga dapat dilakukan upaya pencegahan terhadap ADRs pada pasien geriatriKata kunci: Adverse drug reactions (ADRs), Geriatri, Rawat inap.

Abstract

Background: Hypertension in overweight elderly is a crucial problem considering that its pathogenesis, disease pattern and management are not entirely the same with hypertension in young adults. Hypertension in overweight elderly requires particular attention because it is closely associated with overall management (medical and nutritional).Objective: To study different intake of micronutrients, i.e. natrium (Na), potassium (K), calcium (Ca), and magnesium (Mg) in overweight and non overweight elderly with essential hypertension at Griya Sehat Lansia (GSL) Yogyakarta.Method: The study was analytical with case control study design. Samples were as many as 138 elderly of 60-75 years old taken using multistage sampling technique. Data of intake Na, K, Ca, Mg were obtained through semi quantitative food frequency questionnaire (FFQ); essential hypertension through assessment of blood pressure using sphygmomanometer; overweight and non overweight status through body mass index (BMI), BMI for overweight was 23-24.9 kg/m2 and non-overweight was 18.50-22.99 kg/m2. Statistical analysis used paired t test, Chi-Square and logistic regression.Results: There were differences in intake of Na, K, Ca, and Mg between overweight and non overweight elderly (p<0.05). There were significant association (p<0.05) between intake of Na, K, Ca, and Mg of overweight and non overweight elderly with essential hypertension, with OR 5.271; 6.813; 3.398 and 3.444. Intake of Na and K were variables most significantly associated with overweight and non overweight elderly with essential hypertension (p<0.05).Conclusion: There were significant differences in intake of micronutrients (Na, K, Ca, Mg) between overweight and non overweight elderly with essential hypertension at GSL Yogyakarta.

Abstract

Approximately 10–30% of adult Asian people had metabolic syndrome. This study aimed to reveal effects of workplace health promotion (WHP) with multilevel interventions on workers’ metabolic syndrome component. This study was conducted in 2014-2015 using quasi-experimental design with multilevel subject intervention in Indonesia. The WHP program for multilevel intervention group included 12 weeks of combined physical training, diet, health education, social support, and advocacy. The WHP program for control group included only health education. Instruments used included IPAQ, table of 24-hour food recall, physical measurement tools, and clinical laboratories. Data analysis used Marginal Homogeneity, paired sample t-test, Mc Nemar, and Wilcoxon test. WHP multilevel intervention could improve physical activity and the nutrition in accordance with diet of workers, in particular to increase the amount of fiber and a reduce cholesterol intake. The improved metabolic syndrome components due to the influence of multilevel WHP were systolic and diastolic blood pressure, and fasting blood glucose levels (p value < 0.05). Overall, workplace health promotion multilevel interventions are effective for the management of metabolic syndrome components.AbstrakSekitar 10-30% orang Asia dewasa mengalami sindrom metabolik. Penelitian ini bertujuan untuk mengungkap efek Promosi Kesehatan di Tempat Kerja PKDTK dengan intervensi multilevel pada komponen/penanda pekerja yang menderita sindrom metabolik. Penelitian ini dilakukan pada tahun 2014-2015 dengan menggunakan desain quasi experimental dengan intervensi subjek secara multilevel di Indonesia. Program PKDTK untuk kelompok intervensi multilevel berupa kombinasi latihan fisik, diet, pendidikan kesehatan, dukungan sosial, dan advokasi salama 12 minggu. Program PKDTK untuk kelompok kontrol hanya pendidikan kesehatan. Instrumen penelitian seperti IPAQ, table food recall 24 jam, alat pengukuran fisik, dan laboratorium klinis. Analisis data menggunakan Marginal Homogeneity, paired sample t, Mc Nemar, dan uji Wilcoxon. Intervensi PKDTK multilevel dapat meningkatkan aktivitas fisik dan asupan makanan yang sesuai dengan diet sindrom metabolik, khususnya untuk meningkatkan jumlah serat pangan dan pengurangan asupan kolesterol. Komponen sindrom metabolik yang membaik karena pengaruh PKDTK secara multilevel adalah tekanan darah sistolik dan diastolik, serta kadar glukosa darah puasa (nilai p < 0,05). Secara keseluruhan, intervensi PKDTK multilevel efektif untuk pengelolaan komponen sindroma metabolik pada pekerja.

Abstract

Background: In general, patients with osteoarthritis require long live treatments, especially anti-infammatory drugs. Non steroidal anti infammatory drugs are mostly follow by some side effects such as dyspepsia and gastrointestinal bleeding. The use of natural medicine for rheumatic diseases have commonly been practiced worldwide.Objectives: To learn the changes of hemoglobin level due to treatment with curcuminoid from Curcuma domestica Val. rhizome extract compared to diclofenac sodium as anti-infammatory agent for the treatment of osteoarthritis.Methods: This research is a prospective randomized open end blinded evaluations (PROBE). Patients treated with 30 mg curcuminoid from Curcuma domestica Val. rhizome extract or 25 mg diclofenac sodium three times daily for 4 weeks respectively.The hemoglobin level was checked before and after treatment. Results: A total of 80 patients with knee osteoarthritis participated in this study. There was no signifcant difference in the frequency of sex, educational level, duration of suffering, percentage of co-morbidities in both groups. There was no signifcant different of hemoglobin level before treatment between both treatment group. The hemoglobin level was increase signifcantly in curcuminoid treatment groups while no signifcant change in diclofenac group. Treatment with curcuminoid increasing the hemoglobin level signifcantly compare to diclofenac sodium (p=0.03).Conclusion: Treatment with curcuminoid from Curcuma domestica Val. rhizome increasing the hemoglobin level signifcantly compare to diclofenac sodium treatment for osteoarthritis.
