Abstract

Background: Washing hands with soap on right a time is aneffort to change bad attitudes which does not support theexpected health’s status. Washing hands with soap couldn’tsucceeded if there was not clean water access. This analysispurposed to know the relation between clean water accesswith washing hands with soap’s attitudes on household withchildren under 5 years based on region’s type in Indonesia.Method: This analysis used chi-square’s and log-regression’stest with Riskesdas 2007 data as analysis units. It showedthat 243.396 children under 5 years on 104.109 urban’shouseholds and 139.287 rural’s households.Result: The result showed that the clean water access 90%either in urban or rural was good, but the attitude washinghands with soap were still low, under 60%. Chi-square testshowed there was a relation between the clean water accessand the outcome level with washing hands with soap’s attitude.Log-regression test showed that tended based on the outcomelevel in avarege and poor households, and did not differsbetween urban and rural.Conclusion: There were relations between the clean wateraccess and outcome levels with washing hands with soap.Washing hands with soap ‘s attitude with clean water or notstill on the low level, either on urban or rural. Household withpoor economic’s and avarege’s status tended to low washinghands with soap’s attitude againts the wealthy one. This studyrecommended to improve health’s education to all community,to understand the importance of washing hands with soapusing clean water for all activities, especially before eating,before preparing the food, after loosen the bowels/babies’care, and after holding the animals.Keywords: clean water access, washing hands with soap,household with children under 5 years

Abstract

Background: Health services by Public Health Centres (PHC)in the era of decentralization experienced a decline in someregion. The important role of PHC in health development is thereason to optimize its performance. PHC’s revitalization whichproclaimed by Health Department has the intend to returningPHC to its concepts, health manpower and facility accordingto based values in Kepmenkes 128/2004. The aim of PHC’srevitalization is to increase PHC’s performance in supportinghealth development.Method: The objective of this study was to describe theimplementation of PHC’s revitalization in supporting healthdevelopment. Study area was elect using purposive samplingdesign that is Jombang and Bojonegoro Districts in East JavaProvince, Cianjur District in West Java province and Sikkadistrict in East Nusa Tenggara Province. Data was collectthrough in-depth interview and focus group discussion andwas analyzed descriptive.Result: The result showed that although socialization of PHC’srevitalization has been done, but not yet understood asexpected by Health Department with priority returning PHC’sfunctions according to Kepmenkes 128/2004. Several effortshas been done by several PHCs in this study to improve PHC’sservices using government funding. Kinds of proposals aboutthe form of PHC’s revitalization were the potrait of the PHC’scondition nowadays. This is the reflection of the localspecification of each region.Conclusion: The recommendation of this study is to implementrevitalization of PHC on public health services with attention onthe PHC’s principals wich describe in Kepmenkes 128/2004Keywords : revitalisation, public health centre

Abstract

This article is a policy review that analyze the indicators indeveloping alert villages which was build by Province HealthOffice of East Java. Using normatif approach the core problemand its characteristics was identified. Fulfilment of the indicatorsin developing alert villages in East Java Province and estimatingthe scores to rank the development of an alert village was thecore problem. Analyzing it’s consequencies, resistance andtrade-off was done using predictive approach.Conclusion of the analysis is that using the indicators willfaced obstacles in deciding the development rank which wasreach by the alert villages. Suggestion of this analysis is touse the main steps in developing alert villages which wasstated in Kepmenkes 564/2006 as indicators. Usingpercentages in estimating indicators to fixed the rank of alertvillages was suggest also.Keywords: indicator, alert villages development, policy review

Abstract

The safeguard triangle with a discussion approach can be use as a strategy in the effort to accelerate maternal mortality rate. This triangle was a recommendation of a study in Sampang and Pamekasan Districts in East Java Province through 2003-2004. The triangle consists of the village midwife, the mentor and the pregnant woman with her family in each of its corners. Through interaction of the triangle’s corners the process of discussion approach was conducted. This safeguard triangle was stated as a holistic service because the pregnant woman was involved as a subject not as an object in the discussion approach which assured woman a safe pregnancy and birth. This safeguard triangle which was conducted at the village level should be supported by the supra system in the role of stewards hipness. The discussion approach should be taken into consideration of a coordination process which was formulated to achieve a self care community for a healthy living.Keywords: safeguard triangle strategy, discussion approach, accelerate maternal mortality rate
