Abstract

 Abstract Pharmacy internship in pharmacies is an important stage to prepare prospective pharmacists as health workers. Knowing the process of pharmacy internship implementation is very important in improving the learning system, recognizing obstacles and providing recommendations for solutions to improve the quality of pharmacy education. This aims to know the description of the process and the obstacle of pharmacy internship in pharmacies. The implementation and constraints of the preceptors during pharmacy internship in pharmacies were collected throughFocus Group Discussion (FGD) involving 18 preceptors in Yogyakarta. The data collected were transcribed and categorised, then analysed qualitatively. The Duration, effective hours, process flow, assessment methods still vary, some weresubjective and undocumented; most do not have written procedures and procedures; and evaluation has not been done consistently also has not been acted upon. Constraints include: timing difficulties; compensation; lack of facilities support; limitations on the number and skills of pharmacists and pharmacy employees; as well as the lack of enthusiastic students. The implementation of pharmacy internship in pharmacies needs to be improved to upgrade the competencies of pharmacist graduates. Keywords: preseptors, focus group discussion, pharmacy internship, pharmacies Intisari Praktek Kerja Profesi Apoteker (PKPA) di Apotek merupakan tahap penting untuk mempersiapkan calon apoteker sebagai tenaga kesehatan. Mengetahui proses pelaksanaan PKPA sangat penting dalam memperbaiki sistem pembelajaran, mengenali kendala dan memberikan rekomendasi solusi untuk meningkatkan kualitas pendidikan farmasi. Tujuan penelitian ini adalah untuk mengetahui gambaran pelaksanaan dan kendala preseptor dalam pelaksanaan PKPA di Apotek. Focus Group Discussion (FGD) melibatkan 18 orang preceptor PKPA apotek di Daerah Istimewa Yogyakarta, dilanjutkan dengan analisa kualitatif.  Durasi, jam efektif, alur proses, metode penilaian masih bervariasi; bersifat subyektif dan belum terdokumentasi; sebagian besar belum memiliki tata tertib dan prosedur tertulis; evaluasi belum dilakukan secara konsisten dan belum ditindaklanjuti. Kendala meliputi: kesulitan pengaturan waktu; kompensasi; kurangnya dukungan fasilitas; keterbatasan jumlah dan keterampilan apoteker dan karyawan apotek; serta kurangnya antusias mahasiswa. Pelaksanaan Praktek Kerja Profesi Apoteker di Apotek perlu segera diperbaiki dan ditingkatkan untuk menghasilkan lulusan apoteker yang kompeten. Kata kunci: preseptor, focus group discussion, Praktek Kerja Profesi Apoteker, apotek

Abstract

Drug distribution is an important process in maintaining efficacy, safety, and quality of a drug after manufacturing process. Good Distribution Practices (GDP) should be implemented in distribution facilities included pharmacies. Drug quality should be main concern until the drug is administered to patients. This study is intended to determine GDP implementation and follow-up steps that should be implemented to pharmacies in Mlati, Sleman, Yogyakarta. It is a descriptive qualitative study conducted on pharmacies by reviewing the application of GDP upon the profile aspects of facilities, buildings and equipment, procurement, reception and storage, distribution, handling of product returns and expire date, etc. Primary data obtained by interviewing head of pharmacists (APA) or vice of head pharmacists using the check-list form. Subsequently, the data are analyzed qualitatively to determine the follow-up steps which should be taken to pharmacies that are not eligible to operate. The results showed that no one of pharmacy absolutely follows the GDP rule, 88.89% of pharmacies should be warned seriously, and 11,11% of pharmacies should be suspended temporarily.

Abstract

Fraksi tidak larut etilasetat yang mengandung alkaloid dari bunga kembang sepatu (Hibiscus rosa-sinensis L.) dengan kadar 0,6% b/v mempunyai aktivitas mukolitik yang setara dengan asetilsistein 0,1% berdasarkan kapasitas menurunkan viskositas mukus. Penggunaan bunga kembang sepatu sebagai obat batuk baru dilakukan secara tradisional. Oleh karena itu dibuat sediaan sirup fraksi tidak larut etilasetat yang mengandung alkaloid dari bunga kembang sepatu. Penelitian ini bertujuan untuk mengetahui formula optimum sirup fraksi etanolik yang mengandung alkaloid dari bunga kembang sepatu menggunakan metode Simplex Lattice Design. Fraksi tidak larut etilasetat yang mengandung alkaloid dari bunga kembang sepatu diperoleh dengan menggunakan Vacuum Liquid Chromatography (VLC). Metode Simplex Lattice Design digunakan untuk optimasi formula sirup fraksi tidak larut etilasetat yang mengandung alkaloid dari bunga kembang sepatu  dengan tujuh formula berdasarkan variasi jumlah gliserin, larutan sorbitol 70%, dan mucilago CMC-Na 0,5%. Sifat fisik sirup diuji untuk mendapatkan nilai respon total (R total) terbesar sebagai parameter formula optimum menggunakan metode Simplex Lattice Design  dengan  software Design Expert® versi 8.0.2. Sirup formula optimum diperoleh dengan proporsi gliserin sebesar 25,376%; larutan sorbitol 70% sebesar 51,985%; dan mucilago CMC-Na 0,5% sebesar 22,639%. Sirup formula optimum dibuat, kemudian diuji sifat fisik selama 4 minggu penyimpanan. Data yang diperoleh dari uji sifat fisik sirup dibandingkan dengan nilai prediksi dengan software Design Expert® versi 8.0.2. Stabilitas fisik sirup formula optimum minggu ke 0 dibandingkan terhadap stabilitas fisik  minggu ke 4 menggunakan uji-t berpasangan. Hasil menunjukkan bahwa sifat fisik formula optimum sirup fraksi tidak larut etilasetat yang mengandung alkaloid dari bunga kembang sepatu tidak berbeda signifikan dengan prediksi untuk kemudahan dituang dan tanggap rasa kecuali  viskositas dan derajat keasaman. Sirup fraksi etilasetat yang mengandung alkaloid  dari bunga kembang sepatu hasil optimasi kurang stabil selama empat minggu penyimpanan.

Abstract

Bunga kembang sepatu (Hibiscus rosa-sinensis L.) secara tradisional digunakan sebagai peluruh dahak. Berdasarkan atas Bioassay Guided Fractionation, fraksi aktif berhasil dipisahkan dan alkaloid merupakan kandungan utama fraksi. Oleh karena itu alkaloid digunakan sebagai senyawa penanda (marker) ekstrak etanol Hibiscus rosa-sinensis L. Nilai viskositas digunakan sebagai model untuk aktivitas peluruh dahak, dengan asetil sistein sebagai kontrol positif. Selanjutnya penetapan kadar alkaloid dalam ekstrak etanol dilakukan secara KLT-Densitometri (n=5), kadar alkaloid dibandingkan dengan kurva baku dari alkaloid (marker) hasil isolasi (Y=12,1360X+2901,4474). Kadar alkaloid dalam ekstrak etanol kembang sepatu (Hibiscus rosa-sinensis L.) sebagai 2,35 ± 0,67 %.

Abstract

Abstract: Virgin Coconut Oil (VCO) is a pure oils that is extracted from fresh coconut meat at low temperatures. The formulation of VCO in cream dosage form could retain skin moisture and improve the acceptability. Xanthan gum is one of the emulsifier that used to form oil in water (O/W) cream. The purpose of this research were to determine the effect of xanthan gum concentration to the physical stability of VCO cream. VCO creams were prepared by emulsifying the oil phase and the water phase with the variation of xanthan gum concentration (2.5; 2.7; 2.9; 3.1; 3.3% w/w). Observation of the physical stability of the cream includes organoleptic, emulsion type, droplets size, viscosity, spreadability, adhesion, and ratio of separation volume was conducted until 7th week, and also the freeze-thaw test for three cycles. The results showed that each formulation could produces homogeneous light brown cream with oil in water type. Increasing of xanthan gum concentration would increases viscosity of the cream so that spreadability decreases and adhesion increases during stability study. Ratio of separation volume increased after three cycles of freeze-thaw test. Cream with 3.3% w/w xanthan gum had the optimum physical stability.Keywords : VCO, O/W cream, xanthan gum, physical stability

Abstract

Liposomes in topical drug delivery preparation is developed because having a good penetration through the skin. The lecithin which is often used in liposome formulations is soy lecithin.  Heating and sonication methods can be done to reduce the size of the particles. This study aims to produce nanoliposom dosage formulations of soy lecithin phospholipids by using a combination of heating and sonication methods to generate liposomes with size < 150nm . The methods of nanoliposomes preparation in this study using a combination of heating 60○C and sonication methods for about 30 minutes in the manufacturing process . The results showed that the preparation of nanoliposomes formulation from soy lecithin can be done by using a combination of heating 60○C and sonication methods for about 30 minutes.

Abstract

ISO 9001:2000 is a standard quality management system that focus on processes and customers, since Februari2008 ISO 9001:2000 have been applied in RSU PKU Muhammadiyah Bantul. The aims of this study are to analyzethe increasing level of internal customer satisfaction before and after ISO 9001:2000 and also to measure the patient’ssatisfaction level to find out whether the patients feel satisfied with the service given by inpatient installation afterapplying ISO 9001:2000. The study was done by using analytic cross-sectional method. Data were collected using total sampling method as many as 377 internal customer for internal customer satisfaction questionnaire, while questionnaire of patient’ssatisfaction level used purposive sampling method as many as 69 respondent. As the result, there was an increasing of internal customer satisfaction proved by the difference of averagebetween before implementing ISO as much as 52,48 and about 72,68 after implementing ISO. There is also showedthat found gap between service quality with patient satisfaction and the sequence of gap is responsiveness that isfollowed reliability, empathy, assurance, and tangibles dimension. Keyword : ISO 9001:2000, internal customer satisfaction, importance - performance analysis, ServQual

Abstract

Medical service companies are getting more competitive so companies have to always improve the service quality. The Pharmacy Unit of Yakkum Emanuel Hospital which is a supporting medical unit is demanded to give an optimal service to their patients. The recapitulation of the complaint box in February 2011 showed that 50% of patients (10 patients) complain on waiting time; 30% of patients (6 patients) complain on the hot air in the waiting room; and 20% of patients (4 patients) complain about when the patients will get their medicine. Regarding those issues, this research aimed to analize the level of patient satisfaction on the service quality at pharmacy unit of Yakkum Emanuel Hospital. This research was conducted with descriptive method. The subjects of this research were outpatient in Yakkum Emanuel Hospital that met the inclusion criteria. The number of the sample was 480 patients that taken with purposive method of sampling. The data were obtained using quetionaire about the quality of service based on SERVQUAL model (referring to tangibles, reliability, responsiveness, assurance, and empathy dimensions) and the data were mapped in cartesian diagram. The result shows that there were negative gap values between the patients’ expectation and perception. The highest gap value was on the assurance dimension followed by responsiveness, reliability, empathy, and tangibles dimensions. In general, patients felt dissatisfied with the performance of the management of the Pharmacy Unit of Yakkum Emanuel Hospital Purwareja-Klampok. Based on the Cartesian Diagram mapping, the management of the Pharmacy Unit of Yakkum Emanuel Hospital should pay more attention to the Assurance and Responsiveness dimensions.Keywords: service quality, satisfaction, SERVQUAL.

Abstract

The Red Sirih Essential Oil (RSEO) which be obtained from red betel leaf (Piper crocatum Ruiz & Pav.) by water and vapour distillation has potential as antibacterial activity. Formulation of an active pharmaceutical ingredient to become a pharmaceutical dosage form can generally modify their activity. The aim of the study is to proof the influence of dosage forms to the activity of RSEO. The study was done by formulating the RSEO to become emulsion, microemulsion and solubilisation dosage forms, and then, their antibacterial activity againt Escherichia coli ATCC 25922 by microdilution method of 1.25% using ELISA reader at 570 nm. The results showed that formulation reduced antibacterial activity. The raw RSEO had a Bacterial Growth Inhibition (BGI) of 66.59%, whereas microemulsion had 49.58%, emulsion had 17.45%, and solubilisation had 0% of BGI, that mean lost its antibacterial activity.

Abstract

Liposomes are a delivery system used in pharmaceutical products and cosmetics. Liposomes have many advantages such as increase stability and efficacy, can be targeted to reduce toxicity and increase accumulation at the target site and are biocompatible.  Preparation of liposomes can be done by conventional or new methods which are still being developed. Conventional methods often require a long time and organic solvents which may be toxic. Heating (Mozafari method) is one of the new methods developed in the manufacture of liposomes without organic solvents. Mixing temperature can affect the physical properties of liposomes. The particle size has become one of the important physical properties because it affects the absorption of the drug. Sonication is an easy method of choice in reducing the size of liposomes. Optimization of mixing temperature and duration of sonication in liposomes’ preparation using new heating methods and sonication were performed by factorial design with 2 factors and 3-levels to obtain optimal liposome size. Data were analyzed with two-way ANOVA. The results showed that both mixing temperature and sonication duration significantly affect liposome size, but the interaction was not statistically significant. Data analysis also showed that mixing temperature, sonication, and their interaction do not affect the polydispersity index of liposome. Results showed the optimum mixing temperature and sonication duration that can produce liposomes with size below 100 nm is at 60°C for 30 minutes.
