Abstract

Background and Objective: Anemia is a major health problem affecting every phase of life. However less attention given to lactating mothers where anemia can give bad impacts to the mothers themselves and their babies. Less knowledge also known about nutrition and non-nutrition factors related to anemia. Therefore, this study aims to profile nutrition and non-nutrition factors related to anemia status among lactating mothers.Method: This cross-sectional study was conducted in Grogol Petamburan and Cilincing Public Health Centre in Jakarta in February?April 2019. Seventy fours lactating mothers aged 20?35 years old who delivered within the last 3?6 months recruited using consecutive sampling method. Nutrition factors examined were energy, protein, iron, folate, vitamin B6, vitamin B12, vitamin C intakes, and body mass index (BMI). Non-nutrition factors examined were level of education and family income. Basic characteristic data was collected by interview and dietary intake was assessed using a semiquantitative-food frequency questionnaire and 24-hours food recall for two non-consecutive days. BMI and laboratory assessments (haemoglobin, ferritin serum, and c-reactive protein) were done.Results: Anemia prevalence was 8% and 11% was iron deficient. Prevalence of iron deficiency anemia was 3% (37.5% from prevalence of anemia). Based on Indonesia Recommended Dietary Allowance, around 58% subjects had low iron intake, 39% with low vitamin B6 intake, 27% with low vitamin B12 intake, 40% with low vitamin C intake, and no subject with low folate intake. Around 53% of subjects had BMI < 22.9 kg/m2. Almost 70% of the subjects had middle level of education and 50% had family income lower than regional minimum wage.Conclusion:  Prevalence of anemia among lactating mothers in Jakarta was 8%. Non-nutrition factors had higher proportion compared to nutrition factors related to anemia status. Further studies are needed to determine the possible causes of anemia in lactating mothers.

Abstract

Background:  Early nutritional intervention with partially-hydrolyzed whey-based formula (PHF-W) instead of standard cow?s milk formula (CMF) has been found to reduce the risk of atopic dermatitis (AD) development in non-exclusively breastfed infants with familial heredity of AD.Objective:  To estimate the 6-year economic impact of this nutritional intervention in non-exclusively breastfed Indonesian urban infants with family history of AD.Methods:  A mathematical model simulated AD incidence and burden of using PHF-W vs. CMF in the target population from birth to age 6. The model integrated literature, current cost and market catalogues, and expert clinician opinion. Modeled outcomes included AD risk, time spent post-AD diagnosis, days without flare, quality-adjusted life-years, and costs.Results: Using PHF-W instead of CMF resulted in an estimated absolute 14% (95% CI: 4%, 23%) AD risk reduction, a 0.69 year (95% CI: 0.26, 1.13) per-child reduction in time spent post-AD diagnosis, a 38 (95% CI: 12, 67) increase in days without AD flare, and a 0.046 gain in quality-adjusted life-years. The AD-related 6-year cost estimates when feeding high-risk urban infants with PHF-W were Indonesian Rupiah (IDR) 8,695,057 (95% CI: IDR 4,519,447, IDR13,995,605) and IDR13,139,569 (95% CI: IDR 7,098,794, IDR 19,216,068) per child, respectively, resulting in a net per-child difference of IDR 4,444,512 (95% CI: IDR1,893,080, IDR 8,557,946) favoring PHF-W.Conclusion: PHF-W for the first 17 weeks of non-exclusively breastfed Indonesian urban infants with a hereditary risk of AD demonstrated a reduction in AD incidence, increased days without flare, and increased quality-adjusted life-years and net cost reductions.

Abstract

Background and Objectives:Early menarche predicts higher body mass index (BMI) and adiposity in adult life, and it has been associated with risk factors of cardiometabolic diseases such as elevated triglycerides and waist circumferences also higher risk of adult diabetes. This study aims to explore early-menarche as determinant factor for metabolic-risks among adolescent girls aged 13-15 years old.Methods and Study Design:This cross-sectional study was undertaken in adolescent girls aged 13-15 years in all five municipalities in Jakarta-Indonesia, from January 2014 to Mei 2016. Data collected by using standardized measures for dietary intake, nutritional status indicators, physical activity level, blood pressure, blood glucose and lipid profile, and statistically analyzed by independent-t or Mann-Whitney test, and Pearson or Spearman correlation test.Results and Conclusions:A total of 194 adolescent girls were participated in this study. Early menarche was found in 22.68% of all subjects. There was no association between menarche status and daily macronutrients intake pattern and physical activity level score. Early menarche subjects had significantly higher BMI (p<0.001), CDC-percentile (p<0.001), WHO Z-score (p<0.001), and waist circumference values (p=0.02). Furthermore, early menarche subjects also had higher systolic blood pressure (p=0.035), total cholesterol level (p=0.028), LDL-cholesterol level (p=0.013), and triglyceride level (p=0.026). There was no association between menarche status and diastolic blood pressure, fasting blood glucose, HDL-C level, lipid profile ratio. In conclusion, early menarche is an important determinant factor of metabolic risks, and balance between dietary intake and physical activity level should be prioritized among them.

Abstract

The optimal healthy microbiota during early life still needs further evaluation. Pre- and probiotics are commonly used as supplementation in infant formula.Prebiotic oligosaccharides stimulate the growth of bifidobacteria aiming to mimic the gastrointestinal microbiota of breastfed infants. In general, results with prebiotics in therapeutic indications are disappointing.Studies suggest that probiotic supplementation may be beneficial in prevention and management of disease such as e.g., reducing the risk of necrotizing enterocolitis in preterm infants, prevention and treatment of acute gastroenteritis in infants, etc. Although many studies show promising beneficial effects, the long-term health benefits and eventual risks of probiotic supplementation during early life are not clear.It is likely that ongoing research will result in the use of specific probiotic organisms and/or prebiotic oligosaccharides during the first 1,000 days of life, with the goal to develop a healthy microbiota from conception over birth into the first two years of life with a lowered risk of infections and inflammatory events.

Abstract

Introduction Vitamin B12 plays a role during pregnancy in maintaining folate metabolism. Together with folate and vitamin B6, B12 is involved as a coenzyme in DNA synthesis and various methylation reactions in developing embryos. This study aims to compare plasma folate, vitamin B6 and B12 blood levels with respect to the presence of probiotic strain Bifidobacterium animalis subsp. lactis HNO19 (DR10TM) among pregnant women in Indonesia. Methods: This study is part of a larger study entitled ?Effects of Bifidobacterium animalis subsp. lactis HNO19 (DR10TM), inulin, and micronutrient fortified milk on fecal DR10TM, immune markers, and maternal micronutrients among Indonesian pregnant women.? Further analyses were performed using independent-t test or Mann Whitney test, GLM-repeated measures and chi-square test, to compare folate, vitamin B6 and B12 intake and blood concentration during pregnancy with presence (n=22) and absence (n=55) subjects of fecal DR10TM at third trimester. Results: At the first trimesters there was no difference in plasma vitamin B6 and vitamin B12 levels between the two groups, based on the presence or absence of fecal DR10TM. However, at the second and third trimester, vitamin B6 blood concentration (p=0.034 and p=0.001) and vitamin B12 blood concentrations at the third trimester (p=0.035) were significantly higher in the fecal DR10TM positive group, while having a similar vitamin B6 and B12 intake. Conclusion: Consumption of pre- and probiotics during the periconceptional period may be a useful strategy for improving maternal vitamin B?s vitamins, especially vitamin B6 and B12 status and therefore provide benefits for the offspring?s quality of life.

Abstract

Working in high temperature environment is unavoidable condition for an outdoor worker, especially the outdoor workers in to tropical countries such as Indonesia. Heat stress leads to various heat-related illnesses, such as heat stroke, hyperthermia, heat exhaustion, heat cramps or heat rashes.  A mild and moderate heat stress usually less serious and did not harm general health condition, however it could cause individual fatigue and unfocused, which will interfere the working performance and productivity. Heat stress will had caused physiologic response of the body, as it needs to reduce the increased temperature inside the body; known as heat strain condition.

Abstract

Introduction: The objective of this study was to compare gastric emptying time, hunger response, and blood glucose level after drinking specific oral nutritional supplements (S-ONS) to tea with sugar in healthy adults.Methods: This study was a clinical, parallel, random allocation, and single-blind trial. This study was conducted at Cipto Mangunkusumo General Hospital, Jakarta. The subjects were 12 healthy adults divided into 2 groups: intervention group (n = 6) and control group (n = 6). Patients in the intervention group received 200 mL S-ONS (200 kcal, 18% protein, 20% fat and 62% carbohydrate) and control group received 200 mL of tea with 10 grams of sugar (40 kcal). Gastric volume was measured using 2D-sonography every 30 minutes. Blood glucose level was measured using blood peripheral sample. Hunger response was measured using visual analog scale (VAS).Results: Gastric emptying time in the intervention group was <90 minutes, and in the control group <60 minutes. Blood glucose level was increased in the intervention group and decreased in control group. Hunger response was decreased in intervention group and increased in control group.Conclusion: Gastric emptying time after either drinking S-ONS and drinking tea with sugar in healthy adults subjects were less than 2 hours. Patients receiving S-ONS had significantly higher blood glucose level and were less hungry than the control group.

Abstract

Background and ObjectivesVitamin E is a lipid soluble vitamin which obtain only through diet. Vitamin E has many functions including antioxidant and well known to prevent lipid peroxidation in membrane cell. Vitamin C helps to regenerate vitamin E back into non radical. Vitamin E and C intake among vulnerable group such as lactating mother still uncleared especially in Jakarta. The aim of this study to determine level of vitamin E and C intake among lactating mothers in JakartaMethods60 lactating mothers aged 20?40 years old in 1?6 months postpartum were recruited on Grogol Petamburan and Cilincing Public Health Centre in Jakarta. This cross sectional study held from March 2019 until April 2019. Vitamin E and C dietary intake were collected with semi quantitative food frequency questionnaire. Descriptive analysis was used in this study.ResultsThis study showed that median value  of vitamin E intake was 6.50 (1.20-43.10) gram/day with 91.7% subjects does not meet vitamin E recommended daily intake (RDA) recommendation (19 gram/day), otherwise 70% vitamin C intake was above RDA recommendation (100 gram/day) with median value is 120.05 (23.0-479.2) gram/day.ConclusionVitamin E intake among lactating mothers in Jakarta was not adequate and far below RDA recommendation but vitamin C intake among lactating mothers in Jakarta was adequate and met RDA recommendation.

Abstract

Introduction: Adult individuals in Indonesia showed changes in diet and lack of physical activity, therefore increasing the risk of hypercholesterolemia and obesity. One of nutritional therapy for hypercholesterolemia is rice bran oil, which contains active substances that works synergistically in controlling lipid profile. The substances are gamma-orzanol, phytosterols, and vitamin E isomers (tocotrienol and tocopherol).Methods: A clinical trial was conducted for 4 weeks in City Hall of Jakarta on 20 males, age 1955 years old, with mild-moderate hypercholesterolemia, to assess lipid profile improvement of the supplementation of rice bran oil 45 ml/days compared to 15 ml/days without changing eating patterns.Results: After 4 weeks of intervention, there was significant difference in total cholesterol level between both groups (p = 0.049). In the group that received 45 ml/days of rice bran oil, total cholesterol level decreased 14%, and in the group of 15 ml/days total cholesterol level decreased 7.8%. The reduction of LDL and triglycerides and the increase of HDL were not significantly different between both groups (p >0,05). There was no significant changes of body weight in both groups.Conclusion: This study showed that consumption of 45 ml/days rice bran oil led to better improvements in lipid profiles compare to 15 ml/days. It has been demonstrated that gamma oryzanol and plant sterols in rice bran oil have the capability to remove cholesterol from bile salt micelles, thus decreasing cholesterol absorption in intestine.

Abstract

Everyone, including physicians/doctors knows how to communicate. Communication skill involves how we apply a systematic cycle of sending message, receiving and interpreting it before we give a feedback. However, people only perceived communication as practicing sending the message. And, when receiving a message the feedback-step is the most often neglected. Actually, the feedback-step is the most critical part in which it closes the communication cycle and confirms the understanding of the message.Furthermore, there is a deeper development of communication in which we need to stress into three approaches, i.e. being present, active listening and active empathy. By being present then we can clear the clutter of mental distraction and focus on the immediate communication task at hand. Active listening is needed to apply specific listening-for-understanding techniques that confirm awareness, while active empathy is important to process information for a true awareness of other?s needs. All this approaches take times, of course, however we can anticipate it by practicing effective communication.
