Abstract

Abstract Introduction. Extensive pneumothorax with a long duration of symptoms can be dangerous. Broad pneumothorax can cause hypoxemia which can cause increased production of free radicals in the lung tissue (pulmonary oxidative stress). Oxidative stress in the lungs can indicate the degree of damage or disruption of cells in the lungs due to hypoxia. Methods. There were 4 groups each consisting of 6 mice: 24-hour pneumothoric group (A), 48-hour pneumothoric (B), 72-hour pneumothoric (C) and control (D). Pneumothorac is made by air injection into the right chest pleural cavity, then on an X-ray. All samples were examined for PaO2 to ascertain their level of hypotension. After that, the rats were examined for their lungs malondialdehyde (MDA) levels to find out their stress oxidation stress levels. Results. All groups of pneumothorac mice experienced hypoxemia. The average MDA level was higher in the pneumothorac group, but it was statistically significant only in the 72 hour (C) group with p = 0.031. MDA levels in each group were 1.601 ± 0.739 (A), 1.585 ± 0.714 (B), 2.256 ± 0.513 (C), 1.243 ± 0.162 (D). Conclusion. Pneumothorax can cause pulmonary oxidative stress if the pneumothorac is large in volume and the duration of symptoms reaches 72 hours. MDA can be a marker of cell disruption or injury in cases of pneumothorax who have experienced hypoxemia.

Abstract

ABSTRACT Introduction. Lower urinary tract symptoms (LUTS) is the most common problem in men with age and can reduce quality of life due to various complaints on micturition.   Method. This study was a cross sectional study conducted in May-July 2018 at the Urology Outpatient Clinic, Dr. Mohammad Hoesin Hospital Palembang and 36 samples were obtained. The technique of data collection was done by filling out the questionnaire. Analysis of data in the form of descriptive for general characteristics and correlation analysis used is the Spearman Rank test correlation.   Results: The most prevalence of benign prostate hyperplasia (BPH) occurred in the age group 71-80 years as many as 12 patients (33.3%). BPH patients who came with the most LUTS experienced severe degrees of International Prostatic Symptomatic Score (IPSS) (24 patients, 66.7%). There were 16 patients (44.4%) felt their quality of life was very poor. Spearman Rank correlation test showed no significant relationship between IPSS with the quality of life of patients with BPH with LUTS (p> 0.05, r = 0.073)   Conclusion. There was no significant relationship between IPSS with the quality of life of BPH patients with LUTS.

Abstract

Abstract Introduction: Modified radical mastectomy (MRM) is a breast cancer treatment option that is still operable. One of the postoperative complications that can be found is the formation of seroma. High negative suction drain is done to treat seroma after surgery but it can contribute to increase the length of stay in hospital. Methods: This study was a clinical randomized control trial (cRCT) conducted on 30 breast cancer patients in June - July 2019 at Moehammad Hoesin Hospital in Palembang. This study divided the two sample groups, each group consisting of 15 patients. One group was given half negative pressure on suction drain (experimental group) and the other used full negative pressure on suction drain (control group). Results: There was a significant difference (p <0.005) between the full and half negative pressure groups where there were more seroma events in the full vacuum group in 9 (60%) cases and half vacuum in 2 (30%) but there were no significant differences in long period of stay (p> 0.005). Conclusion: Half negative pressure is more effective in treating seroma than full negative pressure.

Abstract

ABSTRACT Introduction. Gastric perforation is the second most common complication due to peptic ulcer after bleeding where the risk of mortality in patients who have received surgical treatment is still high. The factors that have the greatest influence on the death of the patient are old age and the time span of handling more than 24 hours. This study aims to determine the relationship between risk factors (onset - hospitalization, onset - surgery, increased pulse, decreased systolic blood pressure) on the death of gastric perforation patients undergoing surgery at Dr. General Hospital Mohammad Hoesin Palembang. Methods: An observational analytic study with a case-control study design was carried out from August to October 2017 at the Dr. General Central Hospital Mohammad Hoesin (RSMH) Palembang. There were 90 respondents consisting of 30 sample groups with death cases and 60 sample groups with life control. The relationship between independent and dependent variables was analyzed by Chi Square test while risk factors that play a role in death in gastric perforation patients who performed surgery were analyzed with the Logistic Regression test. Data analysis uses SPSS version 18.0. Results. There was a significant relationship between increased pulse (OR = 11,227 CI95% 3,923-32,129; p = 0,000), onset - MRS (hospital admission) (OR = 4,125 CI95% 1,579-10,773; p = 0,006) and onset - surgery (OR = 3,786 CI95% 1,267-11,308; p = 0.029) on the death of a gastric perforation patient undergoing surgery. With the Logistic Regression test, an increase in pulse and onset - hospital admission was significantly related to the death of a gastric perforation patient undergoing surgery. Conclusion. It can be concluded that the increase in pulse and onset of hospital admission are risk factors that contribute to the death of gastric perforation patients who undergo surgery.

Abstract

ABSTRACT Introduction. Sepsis is still one of the main causes of mortality and morbidity in children in industrialized and developing countries. Pediatric Logistic Organ Dysfunction 2 (PELOD 2) score  aims to assess the degree of organ system disorder. In addition, PELOD score 2 also has a close relationship with death and can detect any organ dysfunction even in patients with low mortality. This study aims to determine the relationship between PELOD 2 score with the death of patients in pediatric surgical cases with clinical sepsis at Dr. Mohammad Hoesin Hospital (RSMH) Palembang. Methods.  An observational analytic study with a cohort design was conducted at the Mohammad Hoesin Hospital in Palembang from August 2017 to December 2017. There were 30 samples of pediatric patients with clinical sepsis who met the inclusion criteria. Data frequency and distribution are explained in tabular form. The relationship between PELOD 2 score (Pediatric Logistic Organ Dysfunction 2) with death was analyzed by the Fisher Exact test. Survival rates are obtained by analyzing survival using Kaplan meier. Data analysis uses SPSS version 18.0. Results. In this study, the results showed that there were no differences in age, weight, height and sex between groups with mortality and life outcomes (p> 0.05). With the Fisher Exact test, it was found that there was a significant relationship between PELOD 2 score and mortality in pediatric patients with clinical sepsis (RR = 13; RR> 2; p = 0.000; p> 0.05). Of 30 pediatric patients with clinical sepsis, there were 13 deaths (43.3%) of pediatric patients with clinical sepsis where 13 out of 15 people (86.7%) in the risk group and the comparison group did not get any deaths. Conclusions. It can be concluded that there is a significant relationship between PELOD 2 score (Pediatric Logistic Organ Dysfunction 2) with the death of patients in pediatric surgical cases with clinical sepsis at Dr. Mohammad Hoesin Hospital (RSMH) Palembang.

Abstract

ABSTRACT Introduction. Secondary infection is one of the most serious complications and the leading cause of death in burn patients. Silver sulfadiazine (SSD) is the standard topical antimicrobial for burns for the past several decades. At present, nano sized silver particles in sulfadiazine nano silver packages have a high surface to volume ratio and remain effective in low concentrations and minimize silver toxicity to tissues. Methods. This study was a prospective parallel open-label randomized controlled clinical trial. This research was conducted in the Plastic Surgery Subdivision of the Muhammad Hoesin General Hospital Palembang Section from May to September 2018. Deep-dermal burn patients were given random therapy of sulfadiazine and silver sulfadiazine nanosilver. Examination of the number of pre-treatment and post-treatment burn colonies was carried out at the Microbiology Laboratory of Muhammad Hoesin Hospital Palembang. Results. Profile of patients with deep-dermal burns at RSMH are male (76.7%), age group between 15-54 years (66.7%), employees (30%), burn area <20% (66.7 %), and caused by fire (56.7%). There were differences in the mean number of pre and post-treatment colonies in the sulfadiazine nanosilver group, but there were no differences in the mean number of pre-and post-treatment colonies in the silver sulfadiazine group. The mean number of post-treatment colonies in the sulfadiazine nanosilver group was lower than the silver sulfadiazine group, but there were no significant differences in the number of germ colonies in the two groups. Conclusion. Sulfadiazine nanosilver is effective in reducing the number of germ colonies in patients with deep-dermal burns compared to silver sulfadiazine.

Abstract

ABSTRACT Introduction. Osteoarthritis (OA) is a degenerative joint disease associated with damage to the joint cartilage and surrounding tissue. Osteoarthritis can affect all types of joints, but generally affects the knee, hip, hand, spine and leg joints. The main clinical features of OA are pain, stiffness and limited mobility. Lequesne index has the advantage of being easily and quickly used to assess the results of new drug research without objective evidence and can be used to follow OA for a long period of time. Methods. This study was a diagnostic study in the form of an accuracy test to assess sensitivity, specificity, positive predictive value and negative expected value. Results. 35 study subjects were obtained according to inclusion. Sensitivity value is 82.41%, specificity value is 71.4%, positive estimation value is 0.92% and negative estimation value is 0.50% and conformity value (kappa value) is 0.80. Conclusion. Lequesne index has a good value of conformity to the radiologic picture with a kappa value of 0.80.

Abstract

ABSTRACT Introduction.  Most of the minor brain injuries will heal as usual, but around 10-15% even 58% experience a continuous or chronic disorder known as post-traumatic syndrome. Post traumatic syndrome has manifested in the first 7 days where the quality of life of patients has decreased significantly. This study aimed to assess post traumatic amnesia in minor traumatic brain injuries in Emergency, Inpatient, and Outpatient RSUP. Moh. Hoesin Palembang using GOAT to determine the classification of minor brain injuries based on post traumatic amnesia and determine the prognosis of mild traumatic brain injury according to the presence or absence of post-traumatic syndrome. Methods. This research is an analytic survey with cross sectional approach. The subjects of this study were patients who were admitted and treated at the Emergency Department, Inpatient Installation, and Outpatient Installation (Polyclinic) at the RSUP dr. Mohammad Hoesin Palembang with mild traumatic brain injury and met the inclusion criteria. Inclusion criteria were patients who present with mild brain injury (GCS EMV score 13-15), age ? 15 years, the time of the event lasts longer ? 24 hours, agreeing and signing informed consent Results. Amnesia assessment uses the GOAT (Galveston Orientation and Amnesia Test) which is divided into three categories, namely PTA free if results> 75, and still in amnesia period if results <66 and 66-75 are doubtful. Based on GOAT assessment the number of Amnesia respondents was 23 people (42.6%) with an average GOAT score or score of 67.6. Conclusion. There is a significant relationship between the incidence of amnesia and post-traumatic syndrome using the GOAT Score assessment.

Abstract

ABSTRACT Introduction. Facial bone fractures can be accompanied by life-threatening complications such as head injuries. Maxillofacial trauma increases with time. The maxillofacial facial injury severity scale (FISS) scoring system was introduced to assess the patient's severity, prognosis, and outcome. Maxillofacial FISS has predictive value on the severity of head injuries. Method. This research uses analytic observation method with cross sectional design approach. The population and sample were all maxillofacial trauma patients who had been treated at RSUP dr. Moehammad Hoesin Palembang from January-September 2018. Data is taken from secondary data, namely the patient's medical record. Results. The incidence of maxillofacial trauma at RSUP dr. Moehammad Hoesin Palembang is 95 cases. The most cases occurred in the age group <30 years (62.1%). Gender male (85.3%), the scene outside the city (52.6%). There were 21 maxillofacial trauma patients undergoing neurosurgery (22.1%). There was no relationship between FISS and the severity of head injury (p = 0.063), there was a significant relationship between FISS and neurosurgery (p = <0.001). Conclusion. There is a relationship between the severity of maxillofacial trauma based on the Facial Injury Severity Scale (FISS) score on the severity of the head injury.

Abstract

ABSTRACT Introduction. In femoral fractures case, bleeding often occurs and is accompanied by injury to other organs, so the possibility of impaired circulation (shock hemorrhagic) will be greater, especially at productive age. This study was aimed to evaluate the benefits of giving pre-operative tranexamic acid to sufferers of femur fractures accompanied by circulatory disorders and without circulation disorders in reducing peri-operative and postoperative bleeding ORIF. Method. This study was a clinical experimental test with a cohort design at the Central Surgical Installation of Dr. Mohammad Hoessin Hospital Palembang. Samples were taken from patients who underwent ORIF Femur surgery, with a type of Hansen Winquist fracture type 0-2, which was pierced with tranexamic acid 10 mg / Kg BW. Result. Intraoperative hemorrhage in ORIF in both treatment groups where patients with a history of preoperative shock had more amount of bleeding than the group who had no history of preoperative shock. Whereas from the amount of post-operative drainage, there were no significant differences in the two treatment groups. Conclusion. The group of patients with a history of hemodynamic disorders (shock) given intravenous tranexamic acid, is useful in reducing intraoperative bleeding, but not significant to reduce postoperative bleeding in both treatment groups
