Abstract

Hypercholesterolemia is complication in type 2 diabetes. Alpha lipoic acid (ALA) reduced total cholesterol by modulatingcholesterol synthesis pathway and reduced cholesterol biosynthesis genes expression. The goal of this research wasto investigate the prohibition of hypercholesterolemia in type 2 diabetes rat by ALA. Fifteen adult male wistar rats (2-3 months, 200-250 g body weight) were segregated into three groups (standard, type 2 diabetes and type 2 diabetes +ALA). Type 2 diabetes induced by streptozotocin 65 mg/kg and nicotinamide 110 mg/kg. ALA was given for 3 weeksafter type 2 diabetes induced. After 31 days total cholesterol of groups labelled is measured. The result showed therewas significant difference between ALA and type 2 diabetes groups (p= 0.012), and there was no significant differencebetween ALA and standard groups (p= 0.591). As the conclusion, this study proves that ALA has effects on totalcholesterol in type 2 diabetes rats.

Abstract

The brain is the central organ of stress adaptation, and is also a target of stress. Chronic stress may result in abnormalchanges in brain plasticity; include dendritic retraction, neuronal toxicity, and suppression of neurogenesis andaxospinous synaptic plasticity. Repetitive stress exposure will gradually change the electrical characteristic, morphologyand proliferative capacity of neurons. Among brain region, the cerebellum is known to be severely affected by oxidativedamage associated with glucocorticoids level. It is believed due to the highest levels of glucocorticoid receptorslocalized in the external granular layer. BDNF, a member of neurotrophin family, is known to be a strong survivalpromoting factor, and plays a critical role in cell proliferation and differentiation, neuronal protection, and the regulationof synaptic function in the central nervous system. BDNF is highly expressed in the cerebellum, mainly in granulecells. Both acute and chronic stress change BDNF expression in the brain. Although the impact of stress on BDNFlevels showed the different results, BDNF is believed to protect neurons from injuries caused by stress.

Abstract

Handwashing is an important thing to do and the most effective way in order to prevent nosocomial infection andinfection control. The most popular surgery?s handwashing is fuerbringer method. The purpose of this study was toknow the effectivity of hand washing from the nurses that work at central operation theather Arifin Achmad GeneralHospital Pekanbaru before and after washing hand with chlorhexidine gluconat antiseptic. This study method waslaboratoric descriptive with 22 samples which got from total sampling method. Bacterial was found from all sample.Bacterial which were found from the samples before hand washing were Coagulase negative staphylococci (CONS)(62.9%), Bacillus sp (20%), Metichillin resistant Staphylococcus aureus (MRSA) (11.4%) and Metichillin sensitiveStaphylococcus aureus (MSSA) (5.7%). Bacterial which found after doing hand wash were CONS (52.9%), Bacillussp (17.7%), Escherichia coli (8.8%), MRSA (11.8%), and MSSA (5.9%). The result of this bacterial colonizationarea showed the reduction of bacterial colonization after doing hand wash in 90.1% sample , which mostly reductionwas happen from II degree to I (36.4%).

Abstract

Accumulation of triglycerides associated with insulin resistance in type 2 diabetes mellitus (T2DM). Alpha lipoic acid(ALA) has hypotriglyceridemic effects by lowering triglyceride levels in blood and liver. This study aimed to analyzeeffect of ALA on triglyceride levels in type 2 diabetic rats. Fifteen Wistar male rats (±8-12 weeks, 200-250 gram) dividedinto 3 groups (control, T2DM and T2DM?ALA given for 3 weeks). Diabetes was induced by giving streptozotocin (50mg/kg.bw) and nicotinamide (110 mg/kg.bw) after 15 minutes intraperitoneally. Diagnosis of T2DM was determined ifthe rats have perifer blood glucose >250 mg/dL after 3 days. The results showed that triglyceride levels in ALA group(p=0,003) lower than T2DM group, T2DM group (p=0,031) had higher triglyceride level than control and no differenceof triglyceride level between T2DM-ALA given group with control (p=0,696). This study concluded that ALA canaffect triglyceride levels in T2DM thus prevents occurrence of hypertriglyceridemia.

Abstract

TNF is important in rheumatoid arthritis (RA) and spondyloarthritis (SpA) pathogenesis. Recently reports showedsTNF levels were significantly lower in SpA than RA synovial fluid (SF). Therefore, the differential regulation of TNFin both diseases were investigated focusing on the balance of sTNF-tmTNF and both TNF receptors expression. Synovialfluid from 22 RA and 25 SpA were analyzed by ELISA. sTNF was confirmed significantly higher in RA than SpA.Total TNF of synovial tissues assessed by qPCR showed similar levels, suggesting a higher tmTNF/sTNF ratio in SpA.The levels of sTNF-R1 and sTNF-R2 were significantly lower in SpA than RA SF. Interestingly, sTNF-R2/sTNF-R1increased in SpA. qPCR on synovial tissue showed similar mRNA levels for TNF-R2 but decrease of TNF-R1 in SpA.IL-6R levels did not showed differences and TIMP-3 tended to be higher in RA than in SpA SF. In conclusion, weobserved a shift from sTNF/TNF-R1 in RA to tmTNF/TNF-R2 in SpA.

Abstract

Talaromycosis marneffei is a mycotic disease caused by Talaromyces marneffei which primarily infectimmunocompromised patient. This disease is endemic in Southeast Asia, and Indonesia consider as endemic area dueto a case of tourist who got the infection after visiting Indonesia. Diagnosis was made based on clinical suspicion withlaboratory confirmation. Clinical manifestations are not specific, such as fever, anemia, weight loss, lymphadenopathy, hepatomegaly, splenomegaly, respiratory disorder, and cutaneous manifestation. Histopathology and culture are themost common examination performed for diagnosis. In histopathology examination, fungi appear as fission arthroconidiacells which shape round to oval, with cross wall formation inside or outside macrophage and histiocyte. Thermaldimorphism characteristic can be observed in fungal culture. Fungi grows as mold at 25oC-30oC and as yeast at 37oC.Definitive identification of T. marneffei was done by molecular examination using primers derived from ITS region asprimary marker and beta tubulin region as secondary marker. Talaromyces marneffei is sapronosis, that transmitted viainhalation of conidia from environment.
