Abstract

Human immunodefficiency virus (HIV) epidemic covers several countries with endemic leprosy without any significantincrease in the number of patients co-infected with HIV and leprosy, so it can be concluded that HIV-positive does notincrease the risk of developing leprosy. Clinicopathological spectrum of leprosy patients who are co-infected withHIV is not changed. Leprosy may be attributed to the presence of immune recovery syndrome after the administrationof highly active antiretroviral therapy (HAART). Long-term incubation period and the low incidence of leprosy inpatients with HIV make prospective cohort or case-control studies on the prevalence of leprosy among a group of HIVpositive and HIV negative hardly possible to be performed. The benefit of this circumstance is the HIV pandemicdoes not become a nuisance of the effort to control leprosy. Further studies on the molecular pathogenesis of M.leprae and other mycobacterial can provide an understanding of the differences of the pathogenesis in patients withHIV infection.

Abstract

Use of antibiotics known is an important factor in the development of resistance tontibiotics, namely the use ofantibiotics that are inappropriate, in the form of overuse or misuse. Inappropriate use of antibiotics should be reducedto prevent the selective pressure that will lead to the development of resistance to antibiotics. Given the magnitude ofthe problem of antibiotics use in hospitals and its impact on the growing and widespread resistance to antibiotics, it isnecessary to immediately improve antibiotic use. Various strategies or interventions can be done at the hospital tooptimize the provision of antibiotics.

Abstract

Heart disease is the major non-obstetric cause of morbidity and mortality in pregnancy Several studies showed thatpregnancy with heart diseases are related to maternal and perinatal complications such as mortality, heart failure, arrhytmias, pulmonary oedema, low birth weight, and prematurity. The aim of this study is to know the incidence, characteristic, and complications of pregnancy with heart disease. This study was using descriptive retrospectivedesign with total sampling technique. The sources of data were medical records of patients in January 2013 – December2017 with number of sample is 37 patients. Cardiac complications was found in 19 (57,6%) cases that was composed ofarrhytmias (24,2%), prematurity (21,2%), pulmonary oedema and low birth weight both 15,2%, and heart failure (9,1%).

Abstract

Provision large amounts of fluids in a short period is known can cause hypervolemia. Therefore, an examination isneeded to find out that the fluid resuscitation being administered does not cause hypervolemia. The purpose of thisstudy is to assess the effect of hypervolemic resuscitation on cardiac contractility. The study was conducted on 10male Sus Scrofa aged 6-8 weeks, as shocked animal models. There are 3 types of resuscitation treatments : normovolemic,hypervolemic-1, and hypervolemic-2. Cardiac contractility was assessed using DPmax and troponin-i levels. There wasan increase in troponin-i levels after hypervolemic fluid resuscitation (p = 0.05). There is a decrease in cardiac contractilityafter hypervolemic resuscitation. Decreased cardiac contractility is associated with increased troponin-i (r = 0.720; p =0.020). Based on the results, we conclude hypervolemic resuscitation causes changes in troponin-i levels, whichreflect changes in cardiac contractility.

Abstract

Mosquitoes act as vector of fatal diseases such as malaria and dengue hemorrhagic fever since a long time ago. Thruits biting, it can transfer diseases from one sick individu to another ‘immunologically naïve’ people. Biting patternsand host preference play an important role in transmission of disease. Those two might differ among species, evenbetween strain from different region. Geographic and some other enviromental play an important role on mosquitoes’sindividual ability. By knowing those factors will built better understanding on the role of mosquitoes on diseasetransmission

Abstract

Patients with urinary catheter have an increased risk to get bacteriuria that could be develop to urinary tract infection(UTI). The aim of this study was to know sensitivity pattern and detection of ESBL in bacteria that isolated from urinof patient with urinary catheter more than 48 hours at Petala Bumi Hospital Pekanbaru. Thirty urin spesiments wasgrowthed in blood agar and MacConkey, incubated for 18 – 24 hours. Identification bacteria was done by some ofbiochemical test, sensitivity test was done by Kirby Bauer method, and ESBL detection done by double disc method.Predominantly bacteria isolated was Gram negative (90%). Gram-negative bacteria was 77.7% sensitive tochloramphenicol and fosfomycin. Gram-positive bacteria was 100% sensitive almost all antibiotic tested. Based onscreening test, this study found E. coli (40%) and Klebsiella sp, Acinetobacter sp and Pseudomonas sp (20%) werepossible produce ESBL, but ESBL was not found based on confirmatory test.

Abstract

One of atherosclerosis risk factor is dislipidemia. One of plant that is expected to overcome dislipidemia is shallot(Allium ascalonicum L.). The aim of this research was to know the effect of shallot’s extraction water on the plasmalipid profile of mice. Groups with treatment consist of yolks group, and shallot water extraction divided into threetreatmen level ; 20%, 40% and 80%. Parameters that were measured were total cholesterol level, LDL, HDL andplasma triglyceride of mice in the end of treatment. Simvastatin and aquadest were separately used as positive andnegative control. This research proves that shallot’s extortion water has effect in decreasing the level of total cholesterol,LDL and triglyceride and also has effect in increasing level of HDL plasma with 80% of concentration as the bestconcentration.

Abstract

The aim of fluid therapy during hemodynamic resuscitation is to increase global blood flow and increase flow to themicrocirculation and therby, and thus oxygen availability for cellular respiration. Effect of volume expansion onmicrovascular perfusion might not always be in coherence with its macrocirculatory. It should be stressed that the totalamount of volume administered during fluid resuscitation is believed to be a major determinant of outcome in criticallyill shock patients. The purpose of this review is to evaluate the boundaries of macrohemodynamic approach to fluidadministration and the integrated with microcirculatory.

Abstract

Squamous cell carcinoma is a nonmelanoma skin malignancy derived from suprabasal keratinocytes. Although completeremission can occur after surgery, patients with advanced stage have poor prognosis. Some cases of high risk canmetastasis. Lymph node’s metastasis occur in approximately 4% of cases with 1,5% mortality rate. Difficulties inidentifying high risk cases and the absence of management guideline are contributing factors in patient mortality.Staging is a process to determine the presence and progression of cancer.
