Abstract

Objective: This study aimed to compare the number of survival, apoptotic and necrotic cells of ipsilateral testicular germinal epithelial cells in male wistar rats with unilateral testicular torsion between nifedipine given and control groups. Material & Methods: Thirty male wistar rats aged 10-12 weeks were randomly divided into 5 groups, each consisted of 6 rats. The negative control group (KN) underwent a sham procedure and left orchidectomy. Positive control group 4 (KP4) and 10 (KP10) performed left torsio testis 3 x 360 degrees medially for 4 hours and 10 hours respectively, then performed orchidectomy 4 hours after detorsion. The 4-hour (N4) and 10 hours (N10) nifedipine treatment group received the same treatment with positive control, but 30 min before detorsion performed, nifedipine were given intraperitoneal 100μg/kg. Within 1 hour after orchidectomy, cell count was calculated using flow cytometry. Results: It was found that the 4 (N4) and 10 hours (N10) nifedipine treatment group had a higher survival cells and also a lower number of apoptotic and necrotic cells compared to the positive control group. It was found that the 10 hours nifedipine treatment group (N10) had a lower number of apoptotic and necrotic cells compared to the 10 hour positive control group (KP10). The difference was statistically significant with p value <0.05. However, in KP4 and N4 group compared with KP10 and N10 group, higher apoptotic cells was obtained. This was a new phenomenon that needs to be investigated more deeply. Conclusion: Intraperitoneal administration of nifedipine prior to testicular detorsion may reduce the number of apoptotic and necrotic cells of testicular germinal epithelial cell, and may increase the number of survival cells in ipsilateral testes with unilateral testicular torsion.

Abstract

Objective: To identify the profile of patients with urinary tract stones at the Urology Department of Soetomo General Hospital Surabaya in January 2016-December 2016. Material & methods: This was descriptive retrospective research conducted at the Urology Department of Soetomo General Hospital Surabaya. The data were obtained from medical records of patients diagnosed with urinary tract stone, with the amount of data collected was 62. The variables included were age, gender, address, main complaint, type and location of urinary tract stone. Results: The ratio of male:female among patients with urinary tract stone is 33:29. Most of the patients with urinary tract stone were aged 46-60 years old (52%), came with the main complaint of flank pain (79%), had the uric acid type of urinary tract stone (48%), and had urinary tract stone located at the kidneys (65%). Conclusion: Profile of patients with urinary tract stone at the Urology Department of Soetomo General Hospital Surabaya is as following: Ratio of male:female among patients with urinary tract stone is 33:29. Most of the patients with urinary tract stone were aged 46-60 years old, came with the main complaint of flank pain, had the uric acid type of urinary tract stone, and had urinary tract stone located at the kidneys.

Abstract

Objective: Prostate cancer is the fourth most common malignancy in men. TRUS guided transperineal prostate biopsy and TRUS guided transrectal prostate biopsy are two main approach to take prostate tissue as diagnostic of prostate cancer. To compare prostate biopsy approach between TRUS guided transrectal and TRUS guided transperineal toward duration of examination, pain perception, and complications. Material & Method: This study was an experimental study with prospective approach. There were two groups, group one was performed TRUS guided transrectal prostate biopsy (TRB) and group two was performed TRUS guided transperineal prostate biopsy (TPB). Evaluation was based on the duration of examination, pain perception, and complication. Data was analyzed using independent T test for duration of examination and Mann-Whitney test for pain perception. Data was performed using SPSS 21.0 version. The statistical significant difference was consider if p value <0.05. Results: There were 20 samples in this study. There was a significant difference in the duration of examination, the average duration of TPB examination (17.40 ± 2.50) was longer than the duration of TRB examination (14.1 ± 2.77). There was no significant statistical difference between TPB group and TRB group in pain perception when USG probe into the anal (p=0.65), anesthesia process (p=0.28), prostate tissue sampling (p=1.00), and post biopsy (p=0.34). Rectal bleeding was found mostly in TRB group (40%) compared to TPB group (0%). Hematuria was experienced by three patients (30%) in TRB group and two patients (20%) in TPB group. Conclusion: TRB was more effective in duration of biopsy than TPB. The complications of rectal bleeding and hematuria were more in TRB group than TPB. The pain perception were the same between both groups. There were no fever, sepsis, hematospermia and vasovagal event in two groups.

Abstract

Objective: Insertion of DJ Stent is a procedure that is often done by urologist. Insertion of DJ Stent can cause LUTS complaints and greatly affect the physical and psychosocial health of patients. The aim of this study was to determine the effectiveness difference of tamsulosin therapy 0.4 mg/day, solifenacin 5 mg/day and the combination of tamsulosin therapy 0.4 mg/day added solifenacin 5 mg/day to manage LUTS complaint after DJ Stent insertion. Material & Methods: This study was a randomized placebo-controlled trial. There were 4 groups, group I received placebo, group II received tamsulosin 0.4 mg/day, group III received solifenacin 5 mg/day, and group IV received combination therapy of tamsulosin 0.4 mg/day added solifenacin 5 mg/day. Evaluation based on International Prostatic Symptom Score (IPSS) and Ureteral Stent Symptom Questioner (USSQ) score. Data were analyzed using SPSS 21.0. It is said to be significant if p<0.05. Results: There were a total of 32 samples consist of 19 (59.3%) men and 13 (40.6%) women. There were significant improvements in the score of total IPSS, IPSS storage and IPSS quality of life score in patients who received combination therapy (p<0.05) when compared with patients who received monotherapy. The highest decrease in USSQ scores 1, 2, 3, 4, 5 and 6 were in the group that received combination therapy when compared with the monotherapy group. Conclusion: The combination therapy is safe and effective to improve IPSS total, IPSS storage and IPSS Quality of Life scores compared with monotherapy.

Abstract

Objective: The patency rates after vasectomy reversal ranges from 71-97%, but there is 26-72% possibility of persistent infertility. Dysfunction or obstruction of the epididymis and oxidative stress are thought to be the important cause of male infertility by disrupting spermatozoa maturation process, causing poor sperm quality. Human erythropoietin or better known as EPO is a glycoprotein hormone that has been purified since three decades ago. Research on the EPO has evolved and become a major research topic the researchers aimed as a therapeutic agent. The cloning and expression of erythropoietin has developed recombinant erythropoietin as a drug that serves as an anti-oxidant, anti-apoptotic and anti-inflammatory. This study aimed to determine the effect of erythropoietin supplementation on sperm motility and morphology in Wistar rat after the release of the vas deferens’ ligation. Material & Methods: Twenty four male Wistar rats were randomly divided into four groups (6 each). On the vasectomy group, the vas deferens were serially ligated for 7 weeks using a non-absorbable suture. The vasectomy reversal group get the same surgical treatment and after 7 weeks the ligation were released. While as in the erythropoietin group, recombinant eryhtropoietin (1000 IU/kg) was administered intraperitoneally three times for 1 week after releasing the ligation. Normal control animals received no surgical manipulation and followed by sperm retrieval for analysis. Eosin-stained slides were prepared to assess the motility and morphology of sperm cells and observed under a light microscope. Results: Ligation of vas deferens significantly decreased sperm motility and morphology. Releasing the ligation of the vas deferens did not improve the sperm motility and morphology. Supplementing eryhtropoietin 1000 IU/kg 3 times for a week after releasing the ligation did not improve the sperm motility and morphology. Conclusion: Erythropoietin supplementation did not improve the sperm motility and morphology in Wistar rat after releasing the ligation of vas deferens.

Abstract

Objective: To determine the effect of Erythropoietin (EPO) on the number of spermatogonia, Sertoli cells, and Leydig cells in white rats wistar strain testis after the release of ligation vas deferens. Material & Methods: Twenty-four Wistar strain rats were grouped into 4 groups. The control group only performed an orchiectomy for testicular examination, ligation group vas deferens only, group performed release ligation of vas deferens, and group performed release ligation of vas deferens and given EPO injection with dose of 1000 iu/kg BW intraperitoneally for 1 week (3x/week). Observation of spermatogonium, Sertoli cells and Leydig cells by counting the amount on the 5 cross sections of the seminiferous tubules using a 400x light magnification microscope with Haematoxylin Eosin staining. Results: Ligation of vas deferens can significantly decreased the number of spermatogonia and Sertoli cells (p<0.05). In Leydig cells there was no significant difference in numbers after ligation of vas deferens (p>0.05). Release of vas deferens ligation turned out to be no significant amount difference in spermatogonia, Sertoli cells, and Leydig cells with ligation of vas deferens group. Similarly, the treatment of ligation vas deferens release and an EPO injection for 1 week was also no significant difference in number compared to the ligation release group of vas deferens. Conclusion: The number of Sertoli cells, Leydig cells, and spermatogonia in the ligation release group of vas deferens and given EPO for 1 week had the same number with the ligation release group vas deferens.

Abstract

Objective: To evaluate the correlation of the weight of the resected prostate with the reduction of the PSA level. Material & methods: This is a prospective study of all BPH patients undergoing TURP procedure in Kardinah Hospital, Tegal, with a timeframe of April–June 2018. Patients consumed α-blocker and 5-α reductase inhibitor medication before the procedure were eliminated from the study. The data collected were the estimated prostate weight calculated using transabdominal ultrasonography (TAUS) of the prostate, PSA level before and after the procedure, and the calculated weight of the resected prostate. The resected tissues of the prostate were examined by an anatomy pathologist and the PSA level will be examined at 1, 14, and 30 days after the surgery. Data was analyzed using repeated measure ANOVA with SPSS version 23.0. Results: We included 35 cases of prostate enlargement in our center. 2.8% of the patients didn’t undergo definitive surgical procedure due to be postponed with various reasons. The patients mean age is 64.2 ± 8.26 with average BMI is 21.8 ± 3.56kg/m2. The median value of the prostate volume is 43.8 mL. The median preoperative PSA level was 8.7 ng/dL, while the median value of the 1st day, 14th days, and 30th days post-operative PSA level were17.67 ng/dL, 6.93 ng/dL, and 3.2 ng/dL, respectively, with significant reduction of PSA level (p<0.001). Conclusion: PSA level post-TURP shows a significant decrease. Every milliliters (mL) prostatic tissue resected could reduce the PSA level for 0.11 ng/mL. This calculation could further be used to predict how much tissue needed to resect, to treat the symptoms, and obtain normal PSA level. In addition, further research is needed, especially with larger sample size and longer follow up period to confirm these findings.

Abstract

Objective: To find a comparison of the effectiveness of ESWT and Tadalafil on organic erectile dysfunction. Material & Methods: This is a Quasi Experiment study that was carried out from February 2018 to April 2018. ESWT treatment was performed in Urology Outpatient Clinic. Cialis 10 mg was used in cased of Tadalafil. The sample size is 40 patients consists of 20 patients each treatment. The data was processed with a significance limit of p<0.05. Results: The sample age average is 61.7 years, the level of total cholesterol is 191 mg/dL, FBG 112.5 mg/dL, 2hPPBG 134.5 mg/dL, HDL 44.9 mg/dL, LDL 137.5 mg/dL, ureum 27 mg/dL, and creatinin 0.8 mg/dL. The proportion of Diabetes Mellitus patients in this study is 50% and hypertension 57.5%, which is 15% of them belong to mild erectile dysfunction category, 37.5% mild to moderate, 5% moderate, and 42.5% severe. We found an increasement of EHS on 75% patients that were treated with ESWT and 5% on patient that were treated with Tadalafil, and it is statistically significant (p<0.05). Conclusion: We found that ESWT and Tadalafil can increase EHS of organic erectile dysfunction patients, which ESWT is more effective than Tadalafil.

Abstract

Objective: Transurethral lithotripsy using Holmium-YAG laser has been reported to be beneficial in breaking up bladder stones with large size (>4cm in diameter) with lower risk of mucosal injury and hematuria. The aim of this study is to evaluate the utilization of Holmium-YAG laser for the management of bladder stones at Kardinah General Hospital, Tegal. Material & Methods: This is a cross-sectional study conducted from January 2017 to March 2017. Patient’s demography, which included age, sex, length of surgery, stone size, and laser’s energy count were recorded. Results: We included 120 patients in this study. Mean of patients age in this study was 51.93 years old with age range were 41-85 years old. Most of the patients were male (109 vs 11) with a mean size of stone 25.09 ± 3.04 mm. Length of surgery ranges from 15 to 75 minutes and mean energy of the laser 28.99 ± 19.34 kJ. There was 100% stone’s clearance following surgery with no major complication occurred. Conclusion: Holmium-YAG laser is effective in managing bladder stones at Kardinah General Hospital particularly for large size stones. Length of surgery and energy of laser used depend on the stone size in which bigger stone size is associated with longer surgery time and bigger laser energy needed.
THE QUALITY OF SPERM AND FEMALE AGE AS FACTORS INFLUENCING PREGNANCY IN INTRACYTOPLASMIC SPERM INJECTION (ICSI) Hadibrata, Exsa; Utomo, Trisula

Abstract

Objective: To evaluate the quality of sperm and the female age influence on fertilization and pregnancy rate after Intracytoplasmic Sperm Injection (ICSI) procedure. Material & Methods: This study was an observational analytic study with a retrospective cohort design. The data of infertile and subfertile male undergoing ICSI for five years were analyzed and evaluated to determine factors influencing pregnancy post-ICSI in Central General Hospital Dr. Sardjito. The Data were analyzed by using Chi-squared test, Kolmogorov-Smirnov test, and Mann-Whitney U test with the confidence level of p<0.05 and α=0.05. Results: There was 52 male included in this study. A total of 52 males were infertile, with azoospermia, oligozoospermia, and oligoasthenoteratozoospermia. 2 patients (3.8%) were patients with oligozoospermia, 32 patients (61.5%) were patients with azoospermia and 18 patients (34.6%) were patients with oligoasthenoteratozoospermia. We evaluated 52 ICSI cycle, the percentage of fertilization and pregnancy are 84.6% and 36.5%, respectively. Bivariate analysis showed a significant relationship between wife under 37 years old variable and pregnancy post ICSI procedure with p=0.021 and OR 9.0 (95% CI 1.05-76.4). There was no significant difference between azoospermia, oligozoospermia, and oligoasthenoteratozoospermia and the occurrence of pregnancy post ICSI procedure, p=0.986. Conclusion: The percentage of fertilization and pregnancy post ICSI procedure were 84.6% and 36.5%. Azoospermia, oligozoospermia, and oligoasthenoteratozoospermia variables were not significantly associated with pregnancy post ICSI procedure. ICSI procedure must be carried out immediately before the age of the wife 37 years because of the high success rate.
